Dietary Accommodations

Does your child have a severe food allergy or disability that requires diet modification?  

PISD Nutrition Services provides dietary accommodations for students with life-threatening allergies and disabilities who have diet modification orders from a physician or other licensed medical authority in the state of Texas. We are unable to provide dietary accommodations for religious or cultural restrictions or individual food preferences.

Students with life threatening food allergies or disabilities requiring diet modifications may have an alert posted in the cafeteria point of sale (cash register) and special menus when needed.  Please have the child's physician fill out the forms below and return to your school nurse. Incomplete forms will result in delays to dietary accommodations being processed.

Please find the following diet modification forms below:
 

  • Diet Modification Form 24-25: Use this form to request dietary accommodations for a student. This form must be completed by a licensed physician or other authorized medical authority licensed in the state of Texas. Please ensure that all required sections are completed or accommodations will be delayed or denied until the required information is completed on the form!
     
  • Remove Accommodations Parent Form: Use this form to remove diet accommodations that are already in place for a student. Parents/guardians may complete this form to remove dietary accommodations and allow their student(s) to be served the full menu at school meals.

Manufacturers provide food labels to Nutrition Services annually.  Product reformulation may occur at any time and may not be known by Nutrition Services.

In addition, distributors may deliver (on short notice), alternative/substitute products which contain unexpected allergens.  Nutrition Services cannot be responsible for ensuring that a child's menu selections are free from allergens.  Students with life-threatening food allergies are encouraged to bring meals from home.
