ADVANCED HEALTHCARE PRACTITIONERS
- CNM - Certified Nurse Midwives

- LAc - Licensed Acupuncturists

Application Process
Below is a list of detailed responsibilities that are need to be completed for applications to be approved by the CMC Credentials Committee and the Board of Trustees.
8 to 6 Months Before Start
- Apply for a Montana license (as early as possible)
- Ensure federal DEA license lists a Montana address
- Get current BLS/PALS/ACLS/NRP certification (If required for your position)
- For Physician Assistants (PAs): complete the State of Montana Controlled Substance Education Course
- Complete background check information link via email from HR
6 to 2 Months Before Start
- Complete online credentialing application promptly after receipt of our office’s notice.
- Complete a signature form for e-signature of electronic credentialing application
- Review these medical staff documents:
- CMC Medical Staff Bylaws
- Rules and Regulations - Provide current malpractice insurance certificates of insurance (COI) if applicant is not covered by Community Medical Center (i.e. employed)
Prior to Clinical Patient Care
- Complete online LifeTalent training:
- Safety Training
- Infection Prevention
- Professionalism in Healthcare
- Cyber Security Awareness Training
- Sexual Violence and Harassment Prevention Training
- Physician and Allied Health: AIDET
- HIPAA 101 - for those new to CMC
- Workplace Violence Prevention in Healthcare - Complete all required Occupational Health Services screenings which may include:
- PPD test (N-95 fit testing, influenza compliance, etc.)
- New applicants may need to submit proof of immunization for Measles, Mumps, Rubella, Varicella, Tdap, and Positive Hepatitis B titer
- Ensure [NPI / NPPES taxonomy code](https://nppes.cms.hhs.gov/#/) and new CMC practice / contract addresses are current and up-to-date
- Coordinate headshot photo with the physician recruiter
- Pick-up I.D. badge from the Medical Staff Office
