The U.S. Passport Acceptance Office at ASU processes completed passport applications and forwards them to the U.S. Department of State for review.

The U.S. Passport Acceptance Office at ASU does not determine issuance, offer refunds or compensate for missed travel.

Two types of payment are required for each application:

  • ASU processing fees and photos are payable with only Visa or MasterCard.
  • U.S. Department of State fees are payable with only a check or money order

Monday through Friday, 8:30 a.m. to 4:00 p.m. by appointment only. The office is closed on university holidays.

Three complimentary parking spaces are available on a first-come, first-served basis.

 

Application process

All applicants must be present during application processing. For children 15 and under, both parents must attend. If they cannot, court documents or a consent form must be provided with a photo ID copy.

Passport application

Proof of U.S. citizenship is required for a new passport. Visit the U.S. Department of State’s website for application details. The U.S. Department of State determines passport issuance.

Passport books and cards

The passport book is globally accepted and may be used for land, sea and air travel.

Passport cards may only be used for land and sea travel between the U.S., Canada, Mexico, the Caribbean and Bermuda. Passport cards cannot be used for air travel.

Renew your passport

Renew by mail

Renew your passport by mail using the DS-82 form if the following applies: 

  • The passport is undamaged, other than normal wear and tear. 
  • You include your current passport with the application.
  • Your passport was issued:
    • In your current name. If your name has changed, document the name change with an original or certified copy of your marriage certificate, divorce decree or court order.
    • When you were age 16 or older.
    • Within the last 15 years.

If any of the above statements do not apply to youapply in person with the DS-11 form.

Renew in person

Complete the DS-11 form before your scheduled appointment and pay a $35 processing fee during your office visit. DS-82 forms are not accepted per U.S. Department of State regulations.

You need to renew your passport in person if the following applies: 

  • The applicant is 15 years or younger.
  • Your passport was damaged, lost or stolen.
  • Your passport was issued:
    • As a limited-validity passport. 
    • More than 15 years ago.
    • When you were 15 years or younger.

Renewal applications for children 16 and under require submission of the most recent U.S. passport and a photocopy of the applicant’s birth certificate.
