Dear Sir/Madam,

I am writing to address a significant concern regarding the security interest/instrument I


tendered to PHH Mortgage Services in February 2022, and September, 2023, and the
circumstances surrounding it. Pursuant to the regulations outlined in Billing Error
Resolution 12 CFR 1026.13, 12 USC 411, 15 USC 1602(H)(j), 15 USC 1635(h), and 15 USC
1692(h), I request that this matter be thoroughly investigated and resolved.

Description of Billing Error: Upon reviewing the terms of the agreement and my recent
interactions with PHH Mortgage Services, I have become deeply concerned about
potential securities fraud and irregularities in the handling of my account. Specifically, I
was asked by PHH Mortgage Services to personally guarantee the application for
services, which I believe constitutes securities fraud. Additionally, I have reason to
believe that the security interest/instrument I tendered to PHH Mortgage Services is
being used in a manner that violates my rights as a consumer.

Action Requested:

1. Conduct a thorough investigation into the security interest/instrument I tendered


to PHH Mortgage Services on November 12th, 2006, and provide detailed
information regarding its usage and implications.
2. Clarify the terms of the agreement and provide documentation that clearly
outlines the rights and obligations of both parties.
3. Immediately cease any actions that may constitute securities fraud or violate
consumer protection laws, including the improper handling of my account and its
associated security interest/instrument.
4. Provide documentation of any adjustments made to my account and ensure that
I am not held liable for any unauthorized charges or fees resulting from this
matter.

I expect a prompt and transparent resolution to this matter, as required by law. Please
acknowledge receipt of this notice within five business days and provide an estimated
timeline for the investigation and resolution process.

If necessary, I am prepared to seek legal recourse to protect my rights and interests as a


consumer. I trust that PHH Mortgage Services will take this matter seriously and work
diligently to address my concerns.

resolution.
