    I have been working as Graduate Engineer Trainee in your organization since


    August 2008. I am thankful to you for giving me an opportunity to work in
    your esteemed organization.

    With due respect, I would like to inform you that, because of my Health
    problem, I have to take treatment in my home town. Hence I am unable to
    continue my services as Graduate Engineer Trainee.

    I am requesting you to relieve me from my duties at the earliest.
