“PACE” stands for “Program of All-inclusive Care for the Elderly.”

PACE is a nation-wide Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.

PACE covers all Medicare- and Medicaid-covered care and services, and anything else the health care professionals in your PACE team decide you need to improve and maintain your health. This includes prescription drugs and any medically necessary care.

Here are some of the services PACE may cover:

• Primary care (including doctor and recreational therapy nursing services)

• Dentistry

• Emergency services

• Home care

• Hospital care

• Laboratory/x-ray services

• Meals

• Nutritional counseling

• Occupational therapy

• Physical therapy

• Preventive care

• Social work and behavioral health services

• Transportation to and from you center for activities or medical appointments

You’ll get your Part-D covered drugs and all other necessary medication from the PACE program. If you join a separate Medicare drug plan while you’re in the PACE program, you’ll be disenrolled from PACE.

PACE Southeast Michigan exists to provide an alternative to older adults who are eligible to live in a nursing home but wish to remain living in the homes and communities they love.

We provide comprehensive care and services in your home, community, and day health centers to help you age on your terms.

You can join PACE Southeast Michigan, even if you don’t have Medicare or Medicaid, if you:

• Are at least 55 years old

• Are able to live safely in the community with the support of PACE Southeast Michigan

• Need a nursing home-level of care (as certified by the State of Michigan)

• Live in our designated service area

If you have Medicaid, you won't pay a monthly premium for the long-term care portion of the PACE benefit.

If you don't qualify for Medicaid but you have Medicare, you'll pay:

• A monthly premium to cover the long-term care portion of the PACE benefit

• A premium for Medicare Part D drugs

There's no deductible or copayment for any drug, service, or care your health care team approves. If you don't have Medicare or Medicaid, you can pay for PACE yourself.

Individuals who do not qualify for Medicaid are responsible for the portion of the monthly premium Medicaid would pay.

If a person meets the income and assets limits to qualify for Medicaid, the program pays for a portion of the monthly PACE premium; Medicare pays for the rest.

Most PACE Southeast Michigan participants come to our day center two or three days a week, while others may attend five days a week or only a few days a month. Your care team will help create a plan with you to determine what is best for you and your individual needs.

All prescription and non-prescription drugs, along with all office visits deemed necessary by the interdisciplinary team, are covered 100 percent by PACE Southeast Michigan. As long as you are enrolled with PACE Southeast Michigan, you will never experience an out-of-pocket cost for health care.

You’ll get your Part-D covered drugs and all other necessary medication from the PACE program. If you join a separate Medicare drug plan while you’re in the PACE program, you’ll be disenrolled from PACE.

While our Day Health Centers are open Monday through Friday from 8:00am to 5:00pm, we provide 24/7 care through our partnership with MEDSTAR. After hours, on weekends, or on holidays, PACE care is just a phone call away.

PACE Southeast Michigan provides transportation to and from your day health center and all medical appointments. A specially equipped bus picks you up at home and brings you to the day health center. Families may also choose to transport their loved ones to PACE Southeast Michigan.

You are free to dis-enroll from PACE Southeast Michigan and resume your benefits in traditional Medicare/Medicaid programs at any time.
