However, there are marked spatial inequalities in healthcare access in Ghana (Peprah, Budu, Agyemang-Duah, Abalo, & Gyimah, 2020; Stewart et al., 2016; Yawson et al., 2017). Recently, a number of initiatives have been made to lessen these access to healthcare disparities. Prominent among these are the free maternal health care program (Witter, Adjei, Armar-Klemesu, & Graham, 2009; Azaare, Akweongo, Aryeetey, & Dwomoh, 2020; Dalinjong et al., 2018a, Dalinjong et al., 2018b), the national health insurance scheme to eliminate financial obstacles that restrict access to and utilization of healthcare (Dixon, Luginaah, & Mkandawire, 2014; Witter & Garshong, 2009), and the construction of health infrastructure to serve deprived areas. Despite these interventions, available evidence has established a strong relationship between socio-economic inequalities and healthcare access and utilization in Ghana (Fenny, Asuman, Crentsil, & Odame, 2018; Kuuire, Bisung, Rishworth, Dixon, & Luginaah, 2016; Novignon, Ofori, Tabiri, & Pulok, 2019). My heart wrenched as I watched this video. It was a hard video to watch, especially when Andrew answered the question: Do you want to die at home or in a hospice?" A powerful reminder of hospices' vital work in providing compassionate care during difficult times. I asked myself what was it like for Andrew to make that video. In those first few seconds, you could see a change in his expression as he answered, realising that whatever brought him to St Francis Hospice - who are very good, by the way - his situation was undeniably real. It's a tough place to be. Watching this video made me reflect on how often we are encouraged to be grateful for even the slightest things. While this is a good practice, I invite you to take this further. Consider this: Are you noticing what you are constantly thinking about? Are you replaying past situations, incidents or comments that hurt you? Do you continue to relive it? Because stress is widely recognised as a significant factor contributing to many illnesses. Notice what you are thinking about and ask yourself if those thoughts are serving you to live a happier life. Is it worth it? As morbid as it sounds, the Stoics had a phrase worth remembering: memento mori—remember you will die - as a profound reminder of our mortality and the impermanence of life. I repeat it to myself whenever I notice I'm making a big deal out of nothing. Make the most of your week ahead.

The compromise that birthed Medicare and Medicaid continues to metastasize. Advocates for Medicare could not get Medicare alone and neither could those promoting a program for the poor. So both got together and Medicare and Medicate was the result, but this only happened when certain states demanded and go the ability to limit the costs. NACHC has long noted deficits of costs greater than revenue for Medicaid patient - their dominant patient type at 50%. Now we see the worst result of the compromise - deficit spending, CHCs held hostage by Congress, cost cutting dominating other public and private plans, and the increasing needs driven by populations increasing that are most behind. Teaching CHCs involve residency training in CHC locations - but they cannot help because Medicaid and HRSA payment designs are the major limitation to health professionals, more delivery team members, better delivery team members, and better practice environments (less turnover and turnover cost, etc.)
