A ventricular septal rupture (VSR) is a cardiac condition characterized by a tear or hole in the wall between the heart's lower chambers (ventricles). Never get bored in the EP room. Case of the day: AFL in heart trasplant patient. First challenge: basal and lewis EKG are not useful (low voltage P and isoelectric line); the CS cannot be crossed (ostium too much distant for the curve of the catheter). Second challenge: verify whether the donor atrium and the recipient atrium are dissociated from each other or not. If they are dissociated we can proceed as with normal heart (often in atrial flutter the cardiovascular surgeon is both innocent and culpable, in this case he was not culpable!). Third challenge: being able to map the entire donor atrium to avoid losing a portion of the tachycardia cycle and falling into a trap (in this case we were helped by intracardiac ultrasound). Once the challenges have been overcome, now we need to burn the isthmus of a "simple" CCW Flutter! The cardiologist passes a thin hollow tube, called a catheter, through an artery and carefully moves it up into the heart. X-ray images help the doctor position the catheter.once the catheter is in place, dye (contrast material) is injected into the catheter.
