Dayana Espinoza and Dajai Foy have successfully completed extensive training, including didactic coursework and a hands-on apprenticeship. They are now fully equipped and certified to embark on meaningful careers in healthcare, addressing critical workforce needs across Texas. After completing the program and passing the exam for certification, Ms. Espinoza works in Neurosurgery and Ms. Foy works in Family Practice. New units are like new shoes. It starts with lots of niggles that need to be ironed out, then gets more comfy. Luckily, I have help as it's me and Jordan (first-year tACP) with some other clinicians A bit about him…. Jordan came to us in 2022 as an ECP in minors. He had done all the courses but hadn't had injury exposure. He said at the time he wanted to be a tACP… We supported him with portfolios and internal XR teaching, and he flew. So, here we are, tACP in acute medicine. The best bit is he's come through our system. For me, that's a success good for retention, and we get great people. We create the conditions, and they grow! We have a system that can facilitate B6-8c strategy from tECP/ENP—>Lead ACPs… He's @UniBham_Nursing on an apprenticeship levy doing his MSc. However, we train our ECPs to L7 with prescribing. This means he can APEL credits across to his MSc. This gives him a six-month hiatus in year two when we can focus on portfolios and fun stuff… We've made it to lunchtime—a good mix of patients, one home, two waiting imaging and two for other results. We are learning as we go today, so we have a slightly slower pace- doing OSCE and documentation practice as we go. ❤️👍 Wowzers…. That day took off. We ended up seeing 11pts in 8hrs with one admission. That 10% ish conversion rate is about average for our SDEC. Great new space. We need to tweak some of our systems now we are in. 👍

This article in the BMJ on medical apprenticeships https://lnkd.in/epZyxkEQ. Started me musing on themes from some of my earlier post on the NHS Workforce plan and considering the evolution of human knowledge and wisdom and learning. Perhaps it started as dispersed hyper-localised knowledge accrued in a single lifetime - the village or tribal healer from a clinical perspective. A paradigm shift in travel or a once in a lifetime migration or the advent of written communication triggers an explosion of composite knowledge. A gradual coalescence into a centralised institution that many cultures would recognise as a "University" allows the knowledge to become centralised and recorded, and lots of positives occur - cross pollination of ideas and concepts, academic rigor and challenge, intellectual and experimental processes evolve and knowledge becomes vast and refined; however lots of negatives occur in these Citadels of learning - they become tools for financial gain, of ego and reputation and power, ring fencing knowledge and controlling the "price" of admission. Similar events may occur in the "quality assurance" sector with Royal and National Colleges. Whilst these Citadels of learning have undoubtedly enhanced humankinds knowledge base perhaps the next stage is the democratisation and globalisation of that knowledge? Of lowering the "price" of access whilst maintaining the quality of "success". How many brilliant doctors has humanity missed out on because the price of access was too high or too far away? How many gifted and caring nurses are not in nursing because the price of access is too high? Is the next paradigm shift in the hands of the visionaries who dare to democratise knowledge and wisdom through technology, innovation and a positively disruptive mindset?
