The risk of early heart failure after heart transplantation is lower if the donor heart is stored in a so-called heart-in-a-box instead of in the usual cooler with ice. This is according to a study where researchers at the Sahlgrenska Academy at University of Gothenburg are now presenting the results. Goran Dellgren is Professor of Transplantation Surgery at Sahlgrenska Academy at the University of Gothenburg, and a cardiothoracic surgeon at Sahlgrenska University Hospital and the Swedish Principal Investigator of the study, responsible for about twenty participants: “The results represent a breakthrough in transplantation, which could expand the field. Used correctly, the heart-in-a-box can reduce a number of complications that otherwise often result in suffering, poor outcomes, in the worst cases premature death, and also high costs,” he says. “In terms of time, organs can also be shipped longer distances, thus making more organs available for transplantation. It is also likely that we can start using less ideal organs from older donors, which could increase the number of heart transplants,” concludes Göran. Researchers in the thoracic immunobiology laboratory in the Division of Cardiothoracic Surgery have diligently studied the mechanisms concerning the immune system's response to organ transplantation. In a recent study, published in the Journal of Clinical Investigation, lung transplant surgeon Ruben Nava, MD, and colleagues within the laboratory investigated the body's immune response that occurs in lung transplantation and the signaling proteins responsible for the development of ischemia-reperfusion injury, a phenomenon linked to organ rejection. Introduction of Organ Transplantation (1954) Overview: The first successful human organ transplant, a kidney transplant, was performed by Dr. Joseph Murray and his team in 1954 at Peter Bent Brigham Hospital in Boston. The surgery involved transferring a kidney from one identical twin to another, minimizing the risk of rejection. This pioneering surgery demonstrated that organ transplantation could be a viable treatment for end-stage organ failure. Impact: Organ transplantation has since become a critical and life-saving procedure for patients with failing organs, including kidneys, livers, hearts, and lungs. Advances in immunosuppressive drugs have made it possible to transplant organs between unrelated individuals, further expanding the potential of transplantation. Although the results are promising, more research is needed to confirm their efficacy, especially in light of new organ preservation techniques such as machine perfusion. To further reduce DGF, several factors have been identified: minimizing organ damage during surgery, using advanced preservation methods, optimizing anti-rejection medications, and potentially using new treatments for both donors and organs. In the TRANSMET trial, 94 patients with liver-only unresectable colorectal metastasis were randomly assigned to liver transplantation (LT) or no LT after a median of 20 cycles of chemotherapy. In preliminary results available in abstract form only, LT improved overall survival, and 40 percent of LT patients were ultimately disease free. Current LT protocols require patients to have stable liver disease on therapy for at least 6 months and no extrahepatic disease for at least 12 months. Urethral stricturing is a narrowing of the urethral lumen as a result of ischaemic spongiofibrosis. The main challenge of currently available treatment options is recurrence of the stricture. Recent advancements in the treatment of urethral strictures mainly came from the fields of regenerative medicine and tissue engineering. Research efforts have primarily focused on decreasing the recurrence of stricture after internal urethrotomy and constructing tissue-engineered urethral substitutes to improve clinical outcomes of urethroplasty surgeries. Glad to see this one out! You would think that this is just another spinal intramedullary ependymoma (IE) dataset like many before. However, the focus of this study was on ‘excellent’ neurological outcome, rather than ‘good’ neurological outcome, showing that full neurological recovery was associated with early surgery in oligosymptomatic patients with suspected IEs. In addition, we identify a subgroup of patients with IEs that show neurological deterioration despite undergoing surgery under ‘optimal conditions’, highlighting the possibility that a more aggressive tumor biology might underly these differences, rendering tumors in this subset more diffuse in their growth and hence highly challenging in their resection. https://lnkd.in/eJjEkvHA
