🔥 Hot off the press 🔥 Happy to share our just published systematic review on Transcatheter valve‐in‐valve interventions after aortic root replacement in Catheterization & Cardiovascular Interventions 🗞 . - The use of stented bioprostheses simplifies procedures due to the presence of radiopaque markers at the site of THV implantation in most commercially available bioprostheses, while this advantage is compromised when stentless or homografts valves are used. - Performing ViV‐TAVI presents specific difficulties in cases of prior VSARR or stentless bioprosthetic ARR, particularly in the setting of aortic regurgitation, due to the absence of a frame or structural support for THV anchoring. - Complications associated to ViV-TAVI like coronary obstruction and annular rupture have to be carefully evaluated. - ViV‐TAVI after ARR in properly selected patients was associated with a low rate of procedural complications and limited but good midterm hemodynamic and clinical outcomes. let's make longevity simple by defining some key terminology: Lifespan is the number of years we live Healthspan is the percentage of lifespan lived in "good health" (physical, cognitive, emotional). Free of disease, frailty, and chronic pain. Sickspan is the percentage of lifespan characterized by frailty and disease. Our conventional medical system is very good at extending lifespan. Think cardiac bypass surgery, antibiotics, chemotherapy, other LIFE-saving technologies). Unfortunately, it's less good at preserving our health in the process. Therefore our sickspan is ever-expanding The goal of longevity medicine is 2-fold: Maximize healthspan so that it fills 99.99% of lifespan Minimize sickspan to the tiniest fraction of a percent of life When you focus on interventions that do the above, increased lifespan typically comes as a side effect Is that really such an outlandish goal?

 ☝ Interesting case of acute abdominal hemorrhage, beautifully illustrates the current role of interventional radiology in cases of acute abdominal bleeding of any kind. ✅  Total procedure time was 45 minutes, with the pseudoaneurysm excluded from circulation after 15 minutes. ✅ As first, superselective embolization of the feeding artery using Onyx 18. Then, due to multifocal, persistent microbleeds, superselective embolization of individual feeding arteries using microcoils Avenir by WallabyPhenox and Cosmos by MicroVention-Terumo. On the control DSA, all suspicious bleeding sites were treated.   ☢ Question for you: 💡 What do you think is the cause of spontaneous bleeding and rupture of the spleen?  💡 Do you prefer superselective embolization, or would you embolize the entire segment? When/if you are diagnosed with atrial fibrillation in addition to risk factor modification your doctor may want to start you on anticoagulation to prevent stroke. We use a score/model to evaluate your risk of bleeding vs your risk of stroke to help guide a shared decision about starting anticoagulation. Anticoagulation comes in many forms - Direct Oral Anticoagulants and Warfarin/Coumadin are the main drug classes utilized. If you cannot tolerate anticoagulation for various reasons your doctor may recommend left atrial appendage occlusion (which will be discussed in a spectate post). 'Unconventional' #PAE using microspheres and adjunctive coil occlusion. I do not use this approach commonly but mainly in older patients who have severe catheter dependent symptoms.The double whammy of microspheres distally to occluded intraprostatic vessels followed by proximal coil occlusion is to provide a more complete and durable occlusion, downside being inability to re treat in case of recurrence.
