🚨 𝑷𝒓𝒆𝒗𝒆𝒏𝒕𝒊𝒏𝒈 𝑪𝒐𝒏𝒕𝒓𝒂𝒔𝒕-𝑰𝒏𝒅𝒖𝒄𝒆𝒅 𝑵𝒆𝒑𝒉𝒓𝒐𝒑𝒂𝒕𝒉𝒚 🚨 💉 Recently had the opportunity to make a significant impact on patient care by intervening in the management of a patient on 𝒎𝒆𝒕𝒇𝒐𝒓𝒎𝒊𝒏 prior to undergoing a diagnostic coronary angiography with contrast dye. ☠️ As we know, patients on 𝒎𝒆𝒕𝒇𝒐𝒓𝒎𝒊𝒏, especially those with CKD, are at increased risk for lactic acidosis when exposed to IV contrast dyes. 💡 My Intervention: Upon reviewing the patient's chart, I identified the need to 𝒉𝒐𝒍𝒅 𝑴𝒆𝒕𝒇𝒐𝒓𝒎𝒊𝒏 48 𝒉𝒐𝒖𝒓𝒔 𝒃𝒆𝒇𝒐𝒓𝒆 𝒂𝒏𝒅 𝒂𝒇𝒕𝒆𝒓 𝒕𝒉𝒆 𝒑𝒓𝒐𝒄𝒆𝒅𝒖𝒓𝒆. This allowed us to minimize the risk of lactic acidosis and protect the patient’s kidney function. I also ensured the patient received adequate hydration to further reduce the risk of CIN. 🎯 Outcome: The angiography was successfully completed without complications, and the patient's renal function remained stable post-procedure. #ClinicalPharmacy #PatientSafety

Attention all! Let’s discuss Proton Pump Inhibitors (PPIs) like Omeprazole, Pantoprazole, and Esomeprazole. These commonly treat conditions like gastroesophageal reflux disease and peptic ulcer disease. While PPIs are effective, it's essential to use them responsibly. Evidence-based medicine suggests that PPIs should be taken at the lowest dose and shortest duration for the treated condition. Long-term use has been associated with risks such as osteoporosis-related fractures, pneumonia, kidney disease, and vitamin deficiencies. However, some conditions, such as severe esophagitis, Barrett's esophagus, and acid reflux, may necessitate longer-term use of PPIs. It's always reassuring to know that you can consult your healthcare provider to determine the best treatment. As a former speech pathologist working with a laryngologist and now a patient advocate, I've seen many patients on long-term PPIs. Patients must take off PPIs for two weeks to make an accurate diagnosis with tests like a pH probe. The Elliot Lewis Center team presented preliminary late breaking data at CMSC comparing premedication regimens for Ocrevus. The research found patients on Ocrevus receiving a combination of oral cetirizine, dexamethasone, and famotidine had a similar frequency and severity of treatment related symptoms, but less sedation than patients receiving oral diphenhydramine, intravenous dexamethasone, and oral famotidine. The data showed this can be a safe and effective premedication regimen for patients with multiple sclerosis pursuing ocrelizumab. https://lnkd.in/eEnc4q9T
