The Fraser Northwest Division of Family Practice in-person monthly “Ask the Expert” event offers an opportunity for family physicians and nurse practitioners to ask a specialist their burning questions. In a recent session, FNW Division member, Dr. John Yap, shared aggregate COPD measures from the HDC Discover application, sparking discussion. “If you have an average practice, but have 20% of your patients with COPD, there may be some work you need to do to ensure this is accurate. If you subspecialize in only respiratory disorders, your aggregates would obviously reflect this. “What should I expect when my loved one is going to die?” As a palliative care/ICU doctor, I've been asked this question many times. Often, we will have that conversation with families where we're at the point where the patient is more likely to suffer than benefit if we continue treatment. So we focus on comfort. And allow our patients to pass peacefully. The question is, what does that look like? Every situation is different, but for the most part, when we know that what our primary objective is (comfort) patients will die peacefully. In the ICU, for example 1. We will reduce the work of a ventilator 2. We will wean off or slowly titrate off any medications that are supporting their blood pressure 3. At the same time we bring in some medications to make sure that they feel no pain 4. Ensuring they feel no shortness of breath 5. We give them medication to take away any anxiety or worry… All of this is to ensure they don't suffer. Typically, the patient will look more and more like they're falling asleep. Their respirations will become more shallow, and they'll become further and further apart, and they'll look peaceful. As a physician, this is a special time. Being with families and patients to help them navigate through such hard circumstances is a blessing because you're in their world. You learn about Uncle Tommy, and what he was doing at dinners at Thanksgiving. And Aunt Nancy who was kicked out of school for being a rebel. You share in the laughs and in that experience and you get to see the human side. And so when it does come to end-of-life care, it will be peaceful, as long as the goal is comfort as a clinician. It’s challenging but it’s so rewarding. Because you know how hard it is, and you know what you’re doing is so incredibly impactful. If this resonates with you. ICU and Palliative Care: Poles Apart or a Mindset in Common? "All this got me thinking… how do we make the perceived value of excellent end-of-life care equal to that of the neat and tidily charted, all-pumps-in-a-row care of the highly dependent ICU patient? For the intensive care entrenched, there is something so entirely different about removing extraneous equipment, turning off unnecessary monitoring and forsaking the hallowed ICU chart. I always support a proactive approach
