What are the post-acute care preferences of patients and caregivers between home-based and facility-based settings? Researchers found a significant preference for home-based care, with both patients and caregivers preferring personalized, familiar environments during recovery. To continue progress towards patient-centered care in the home, researchers emphasize the importance of ensuring equitable support and addressing the needs of disadvantaged groups in promoting patient-centered post-acute care. https://lnkd.in/gGZYRrdg I #PostAcuteCare #CareAtHome #HomeBasedCare #HospitalAtHome #RemotePatientMonitoring JAMA

In our approach to community support, Invisible-Care Inc. emphasizes the importance of collaboration between digital health and live support workers. We firmly believe that this collaboration has the potential to bring about transformative changes in the landscape of healthcare. While boots-on-the-ground care is undeniably essential, it is often constrained by factors such as the limited availability of human resources, time constraints, and the willingness of clients to welcome someone into their homes. Digital support emerges as a solution that effectively fills in these gaps, providing clients with a comprehensive spectrum of support throughout the day. By integrating live support with digital health solutions, we create a synergy that significantly enhances the outcomes for the clients we serve. #rehabilitation #rehab #braininjury #concussion #chronicpain #spinalcordinjury #mva #catastrophicinjury #accidentinsurance #claimsmanagement #occupationaltherapy #occupationaltherapist #casemanagement #personalinjury #personalinjurylawyer #homecare #longtermcare #communitycare #communityengagement #seniorcare #mentalhealthsupport #healthtech #attendantcare #personalsupport #psw #occupationaltherapist #casemanager #abi #tbi #dementia #behavioralhealth #ondemandservices #seniorcare #claims #catastrophicinjury #sabs #invisiblecare #assistivetechnology #assistedliving #assistivetech #chooseOT #braininjuryassociation #braininjuryconference #braininjurysurvivors #braininjury #dementia #cognitivehealth #remotemonitoring #telehealth #telemedicine #digitalhealth #mentalhealthsupport #mentalhealth #healthtech #healthtechinnovation #pediatrichealth #oap

Since 2012, there's been a 45% 📈 in patients seeking care at Community Health Centers (CHCs), amid ongoing funding worries 💸. In this context, #PacificVisionFoundation, along with #PacificVisionSurgeryCenter, LIONS EYE FOUNDATION OF CALIFORNIA NEVADA INC, and the CPMC Ophthalmology Residency Program, are providing essential sight-saving 👀 services to thousands from CHCs and Federally Qualified Health Centers (FQHCs) across California and Nevada. These centers, grappling with staff shortages and scarce resources, find it challenging to diagnose and treat specialty eye conditions, including vital injections or surgeries 🔍🚑. A notable concern is the high prevalence of diabetes among CHC and FQHC patients, a condition closely linked to early vision loss, highlighting the urgency for prompt intervention. Our united efforts 🤝 aim to guarantee early detection and treatment, lighting a path of hope 🌟 for reversing vision loss and improving life quality for those in need. #VisionCare #CommunityHealth #DiabetesAwareness #SaveSight #HealthcareHeroes #California #Nevada

As the U.S. population ages, non-medical personal care services are increasingly important for supporting Americans to remain in homes, as the vast majority of them prefer. But in-home personal care services will remain in short supply throughout the country unless home care agencies have greater success recruiting and retaining caregivers. In this HMA white paper, authored by HMA Principal Barry Jacobs, we describe the programs developed by Help at Home, the nation’s largest personal care services providers with 53,000 in 11 states, to use technological solutions to increase ease of caregiver recruitment and to provide its caregivers with a greater sense of purpose and meaning in their work. Reporter for Behavioral Health Business. Inquisitive observer of the behavioral health industry. Lifelong learner and truth-seeker.
We learned with some specificity why Optum acquired the outpatient #mentalhealth providers Refresh Mental Health and CARE Counseling. At the time of the Refresh deal, we learned about some vague ambitions about holistic, value-based care. Optum blew off any request for comment about the CARE deal. But earlier this week, the new president of Optum said outright that the reason for the acquisitions was an attempt to address one of the most challenging hurdles facing this slice of #behavioralhealth. #addictiontreatment #managedcare #healthcare #healthinsurance https://lnkd.in/eNhtdwfQ
