As cited by Korah, Nunbogu and Ahmed (2023): “Equitable access to health care for everyone is a fundamental goal of the SDGs (UN, 2015). However, there are marked spatial inequalities in healthcare access in Ghana (Peprah, Budu, Agyemang-Duah, Abalo, & Gyimah, 2020; Stewart et al., 2016; Yawson et al., 2017). Recently, a number of initiatives have been made to lessen these access to healthcare disparities. Prominent among these are the free maternal health care program (Witter, Adjei, Armar-Klemesu, & Graham, 2009; Azaare, Akweongo, Aryeetey, & Dwomoh, 2020; Dalinjong et al., 2018a, Dalinjong et al., 2018b), the national health insurance scheme to eliminate financial obstacles that restrict access to and utilization of healthcare (Dixon, Luginaah, & Mkandawire, 2014; Witter & Garshong, 2009), and the construction of health infrastructure to serve deprived areas. Despite these interventions, available evidence has established a strong relationship between socio-economic inequalities and healthcare access and utilization in Ghana (Fenny, Asuman, Crentsil, & Odame, 2018; Kuuire, Bisung, Rishworth, Dixon, & Luginaah, 2016; Novignon, Ofori, Tabiri, & Pulok, 2019). November is National Hospice and Palliative Care Month and this year's theme is "courageous conversations." Death doulas, or end-of-life doulas, often serve in a supporting role to hospice teams, assisting clients and their families in an on-call or as-needed basis. My heart wrenched as I watched this video. It was a hard video to watch, especially when Andrew answered the question: Do you want to die at home or in a hospice?" A powerful reminder of hospices' vital work in providing compassionate care during difficult times. I asked myself what was it like for Andrew to make that video. In those first few seconds, you could see a change in his expression as he answered, realising that whatever brought him to St Francis Hospice - who are very good, by the way - his situation was undeniably real. It's a tough place to be. Watching this video made me reflect on how often we are encouraged to be grateful for even the slightest things. While this is a good practice, I invite you to take this further. Consider this: Are you noticing what you are constantly thinking about? Are you replaying past situations, incidents or comments that hurt you? Do you continue to relive it? Because stress is widely recognised as a significant factor contributing to many illnesses. Notice what you are thinking about and ask yourself if those thoughts are serving you to live a happier life. Is it worth it? As morbid as it sounds, the Stoics had a phrase worth remembering: memento mori—remember you will die - as a profound reminder of our mortality and the impermanence of life. I repeat it to myself whenever I notice I'm making a big deal out of nothing. Make the most of your week ahead.

Hospice, the word can be scary much like it is a place, an ending or a destination. Really hospice is a journey ,a pathway you take when curative means are either not available or just too difficult to endure. It’s not giving in or giving up but taking a different road. You choose a path of comfort. A path that allows more quality time with family, time where you call home, time to heal emotionally and spiritually while your body is made comfortable and your mind is offered peace. No more procedures and test with bad news. You are never rushed or hurried on this path, just allowed to take this path at your own pace - however long that may take. Hospice is not a destination. Hospice is peace and comfort for you and your family and hospice is for 6 months or more. #wecarewhereyouare #comfort #symptommanagement
