📣 Home Health Leaders CMS is giving a first look at your Discharge Function Score in your IQIES 'Review and Correct Report'. This measure will be publicly reported on Care Compare in January 2025 and will also replace the TNC Measures for VBP. Some things to remember: ◾Your 2024 performance drives your 2026 payment- your strategy is important. ◾Accuracy of the GG items matters RIGHT NOW. ◾The GG DC Goals that are set at SOC/ROC are being removed- these are NOT used to calculate DC Function Score. ◾DC Function Score has a risk adjustment model that predicts an expected DC score, this is compared to the observed DC score. ◾Make sure your clinicians and QA team understand when it's appropriate to use 'NA' codes at SOC/ROC and DC- this is important! Let's talk about 'good enough.' As #physicaltherapists and healthcare professionals across the board we want what is best for our patients. The problem is "the system," (meaning all of the barriers to actual wellness that exist), kind of makes that impossible. The conflict exists in that we know that if we could truly partner with the patient without barriers and give them the support they need to overcome we can likely truly and impactfully change their health outcomes and improve their lives and quality of life. But the system is so downstream, reactive, and trapping for health. This is even more evident in the #emergencydepartment where the time and resources are limited. So, some reminders about that. We can't fix everything. We can make it "good enough." We can make it better even if just a little. Having #PTintheED improves patient satisfaction, outcomes and operations. It's a way for us to give people that extra bit of hope and care in one of the bleakest moments in their healthcare experience. I know it's not bloody good enough. Last week we shared part of a recap of a trial we completed for a 1.5 Star agency in the midwest. We found additional reimbursement despite taking a loss in 2 charts, which is very rare. Here is where we also help with accuracy and begins to explain some of the reasons they are a 1.5 star agency. As you can see, we suggested changes on all 10 charts on M2020 and 5/10 for half of the 14 questions associated with 5 Star Outcome Ratings. If OASIS isn't correct at the beginning, then agencies won't have much of a chance to reflect the improvement and positive outcomes that is likely happening but aren't getting credit for. If clinicians show a patient is a 0 or 1, then they can't improve much at all (when in all likelihood they patient was more than a 0 or 1 prior to whatever event happened for them to need home health care). The patient improves in reality, but doesn't because there's no where to improve on OASIS. Agencies suffer. Referrals can be impacted. Why would a referral source want to choose a low star rating partner- even though actual outcomes are great. VBP outcomes suffer too. Accuracy is the most important. Get it right up front and better things happen for the patient and agency at the end of the episode. #PDGM #HHVBP #oasis #homehealth #homehealthagencies
