This week on BackTable: I recently sat down with Dr. Neil Khilnani to learn about his approach to the management of pelvic venous disorders, including embolization practices for treating conditions such as venous origin chronic pelvic pain and varicosities of pelvic origin. Dr. Khilnani is an Interventional Radiologist at Weill Cornell Medicine. Dr. Khilnani discusses the significance of identifying and treating venous disorders in patients with chronic pelvic pain. He emphasizes the need for a comprehensive approach that includes detailed patient history, physical exams, and imaging techniques such as ultrasound, CT, and MRI for accurate diagnosis. #AGS2024: Rates of intraoperative complications, cystoid macular edema (CME), and visual improvements are similar between patients who receive cataract extraction combined with trabeculectomy (phaco+trab) compared with patients who undergo cataract extraction after prior trabeculectomy (post-trab phaco), according to research presented at the American Glaucoma Society's 2024 Annual Meeting. I have also spent much of my professional business life advising and developing medical technology companies. MedTech is an interesting aspect of healthcare development and investing - it is responsible for quantum and incremental improvements in patient care and the health of patient-serving hospital systems. The following case record (CPC) of my academic alma mater, the Massachusetts General Hospital in the recent New England Journal is worth the read for the extraordinary, sequential progress we have made in the care of critical cardiac disease. 25 years ago an out-of-hospital cardiac arrest, witnessed or otherwise, had no real chance of survival. Ophthopedia Update:Consequences of Clot Formation and Hyphema Post-internal Trabeculotomy for Glaucoma: Precis: Clot formation and hyphema following internal trabeculotomy represent distinct clinical entities. The eyes with clot formation exhibited a more pronounced postsurgical intraocular pressure spike, longer residual intracameral bleeding, and a higher risk of reoperation. Purpose: To investigate the consequences of clot formation and hyphema in the anterior chamber after internal trabeculotomy. Methods: In this retrospective interventional comparative study, we investigated the surgical outcomes of internal trabeculotomy in 142 eyes of 142 patients at Sensho-kai Eye Institute. Results: Concurrent clot formation and L≥2 hyphema (height of hyphema ≥1 mm) was observed in 22 eyes. In these cases, the post-surgical IOP was 29.3 mmHg at 1 week, significantly higher than the 16.1 mmHg in eyes that had L≥2 hyphema but without clot formation (P=0.0002). However, the 1-week post-surgical IOP in L≥2 hyphema and clot (-) eyes, which measured 16.1 mmHg was not significantly greater than that in L #Glaucoma #Ophthalmology #Ophthotwitter

Pattynama on "Endoscopic vacuum therapy for esophageal perforation: a multicenter retrospective cohort study" The efficacy and safety of endoscopic vacuum therapy in 27 patients with esophageal perforations (mostly iatrogenic and Boerhaave syndrome) were investigated. Defect closure was achieved in 89% after a median treatment duration of 12 days and 1 sponge exchange. There were 2 adverse events: 1 iatrogenic defect expansion during sponge exchange and 1 hemorrhage during sponge removal.
