#Precautions_for_high_voltage_related_ #operations. #Ev_bus The operators shall wear insulated rubber shoes and insulated gloves for one-hand operation. The emergency service switch assembly shall be operated preferably by a certificated technician to avoid mis-operation by multiple persons. In the entire vehicle assembling process, the emergency service switch handle must be pulled off and managed by the certificated technician. Before maintenance and low voltage testing of the vehicle, confirm that electrical appliances of the vehicle are OFF and then remove the emergency service switch handle. Before high voltage testing or power-up to high voltage, confirm that the all electrical appliances of the vehicle are OFF and remind the surrounding persons to temporarily keep away from the on-board high voltage equipment, and then plug the emergency service switch handle. In case of emergency, remove the emergency service switch handle immediately and cut off the high voltage power supply of the vehicle. This got me thinking, could only daily inspections have prevented this , or thera are major organizational systemic issues relating to maintenance schedules of equipment ? Normally when incidents occur, the first impulse is to assume that the employee must have missed something, eg in this case possibly not doing pre-start inspections, however a deeper analysis can see that these are symptoms oftentimes of greater management level and systemic issues which are the real / root causes of incidents.
