Without Electronic Health Records (EHR), the patient's request for a password reset would have likely involved a more cumbersome and time-consuming process. The patient might have had to provide extensive information over the phone or through traditional mail, leading to delays in accessing medical care. I whole heartedly agree with Jennifer! A couple questions I have: 1. How do we educate our providers about this and what are the penalties? 2. What do you think about “carry over” template? For example, a copy and paste of previous office visit and then just add to the end of the assessment new or “patient condition remains the same”, with some added new vital signs?
