South Africa grapples with a profound #TB crisis, with 280,000 cases and 54,000 deaths annually, compounded by challenges like high HIV prevalence and limited #healthcare access. Partnering with the KwaZulu-Natal Department of Health, THINK South Africa, and LTE Medical Solutions - a Division of Vertice Healthcare, we have used AI-augmented portable X-ray machines in mobile vans to drive #health equity in the region. By leveraging #AI for X-rays, we are helping rural communities in #SouthAfrica as well as the overburdened healthcare providers. Community outreach initiatives have also been instrumental in encouraging TB screenings, and AI-powered X-rays help identify asymptomatic patients in high-risk areas. We are grateful to see that health officials are enthusiastic about scaling this program and exploring its potential in diagnosing other diseases. In June 2023, MSF and the Mombasa Department of Health launched a study to better understand the health and well-being of young members of 'key populations'. Close to 600 participants were part of this community-led study. ‘Key populations’ refer to a diverse collection of social groups that face unique health disparities, especially in relation to HIV and other sexually transmitted infections. The Mombasa Key Pop Study represents an ongoing collaboration with the diverse key population organisations and communities of Mombasa. As study materials were created (e.g., survey instruments), all community collaborators were invited to review and provide critical feedback. Recruitment of participants to all stages of the study was conducted primarily through key population community organisations. Feedback from these organisations was also used to refine and revise all analyses of study data. The study shows that stigma and discrimination remain pressing issues impacting vulnerable young people's lives, health and well-being. The aim is that these findings will help inform initiatives that help improve the health of these populations. Global health risk researcher from Griffith University, Dicky Budiman, said, when WHO lifted the first global health emergency for Mpox in May 2023, Mpox's status was a silent epidemic or it had not disappeared. "In fact, the Mpox virus continues to circulate and develop, giving birth to mutations with the emergence of the new Clade 1b strain variant which has a severity and fatality rate of up to 10 percent," said Dicky. According to Dicky, Mpox is closely related to human behavior. “Diseases related to sexual behavior like this, similar to HIV, will be difficult to completely eradicate. "We need ongoing treatment, especially risk communication strategy to build community awareness and literacy," he said. However, this does not mean that Mpox can become a global pandemic. According to Dicky, from the context of a pandemic, the condition for a disease to become a pandemic is that it is new. #AI-driven predictions can be a crucial tool in the global fight against HIV. Through Project Beyond – our cross-sector partnership in Kenya – #Pendulum empowers healthcare workers with predictions and risk scores to increase the uptake of prevention commodities while improving patient adherence to ART. “We are able to look at the historical data, and [our AI] is able to tell us that: ‘X amount of patients are at the highest risk’ or who might become lost to follow-up,” Pendulum’s Rene Ishimwe explained at the Global Health Supply Chain Summit in Nairobi. In #Tanzania, the Unified Community System (UCS) is helping transform health service delivery at the grassroots level. Developed through Digital Square at PATH, UCS is specifically tailored to the needs of community health workers and facilitates the identification and enrollment of patients across multiple community health services. It does this by consolidating and capturing information from various community interventions through three inputs: 1) The WAJA App empowers community health workers to efficiently register clients, link them to essential health services, and conduct follow-ups, ensuring no appointment is missed. 2) The Kituoni App streamlines the process of serving clients, ensuring that health care providers have immediate access to relevant patient data and histories. 3) The Dashboard provides comprehensive insights through various indicators and reports, delivering actionable intelligence in addition to data. Ultimately, the UCS is a decision-support tool that helps bridge the gap between community- and facility-based health care. Its primary goal is to identify new cases of illnesses, such as HIV and TB, and to ensure that the patients receive the best possible care. The UCS also tracks missed appointments and links clients to follow-up appointments for a seamless continuum of care and provides decision support to health care providers during critical moments. The system is driven by a commitment to make health care more accessible, efficient, compassionate, and data-driven. UCS was developed through a strategic partnership between Digital Square, the Tanzania Ministry of Health - ICT, National AIDS Control Program, National Malaria Control Program, and key partners including Jhpiego, Deloitte, Elizabeth Glaser Pediatric AIDS Foundation, and D-tree. Will he [or she or they] gain anything by it? Will it restore him [or her or them] to a control over his [or her or their] own life and destiny? Then you will find your doubts melt away." Our health system seems to be following Gandhiji’s Talisman upside down. Instead of serving the most marginalised it is getting more geared to leave them behind. Our health system must meet the needs of the poorest of the poor and weakest of the weak. 🎯 A Case Study: Using Machine Learning Solutions to Improve HIV Treatment Retention in Public Healthcare Facilities 🤔 What is the problem? ART adherence remains a challenge across HIV-burdened countries in Sub-Saharan Africa. 💡Solution? Along with prominent implementing partners in South Africa, we developed a digital tool, the Support Planner, that is trained on patient treatment data to predict the risk of Interruption in Treatment (#IIT). 📚 How are we implementing the solution? The tool assimilates patient follow-up lists generated by data capturers using facility data management system-generated reports. It produces a list categorizing patients as low, medium, or high risk for IIT. The case managers use the list to prioritize high-risk patients and allocate resources accordingly. What are some early insights? ✅Deployed in 16 health facilities. ✅Scored over 24000 patients and identified 32% as high-risk ✅87% of case managers reported time saved in identifying high-risk patients ✅94% of case managers agreed with the risk score What is in the pipeline? The project is in the final stages. In the coming months, we will analyze the findings to measure the impact of the support planner on patient outcomes. Buckle up for 2024 🛩️. Let's delve into the world of public health! Did you know that despite effective interventions, STIs and Viral Hepatitis (VH) persist as major public health concerns? These issues extend beyond physical health, impacting communities and carrying adverse consequences like cancer due to chronic VH infections. In Kenya, services are provided to Key Populations, including persons living with (PLHIV), and the general population, emphasizing the critical role of STI and VH screening, especially for pregnant women. However, there's room for improvement. Fragmented service delivery, inadequate community education, and weak supply chains present challenges. But here is a beacon of hope! Guided by the 2022-2030 Global Health Sector Strategy, countries, including Kenya, are called upon to enhance their response to HIV, Viral Hepatitis, and STIs. Kenya's MoH has taken a significant step by establishing a Technical Working Group (TWG) for Viral Hepatitis and STIs. In the past week my work station has been Shangani were the team conducted an intense 5-day Expanded Access to Radiology Services screening activity aimed at addressing the critical health challenges faced by artisanal and small scale miners in the area. Through meticulous data collection and evaluation, we were able to assess the effectiveness of the screening activity and measure the positive impact it had on the lives of the miners. Some of the key outcomes are: 1. Early detection and ntervention - through the screening techniques and also leveraging on the expertise of healthcare professionals, we identified cases of TBand silicosis at an early stage. This early detection is crucial for effective treatment and prevention of further spread. 2. Access to Care - our screening activity served as a gateway to essential healthcare services for the affected miners. The miners were able to access treatment and we ensured they received the necessary care, medication, and ongoing support. 3. Health and safety education and awareness -to prevent future cases, we also conducted health and safety educational sessions on TB, silicosis and HIV prevention strategies aimed at improving the miners knowledge and to build a sustainable culture of a health seeking behaviour BAINES OCCUPATIONAL HEALTH SERVICES #endtb #silicosis #occupationalhealth #ASMs #HIV #TB
