Titanium-nitride-oxide–coated vs drug-eluting stents in acute coronary syndromes: an individual patient data meta-analysis. #LateBreakingScience at the #ESCcongress2024 In this IPD-meta-analysis of the only three RCTs comparing TiNOS with DES in ACS, we demonstrate that TiNOS, whilst being associated with similar rates of overall major adverse cardiovascular events, was associated with significantly lower rates of cardiac death, myocardial infarction, and stent thrombosis as compared to DES. These findings have significant implications for current practice as they call into question the default use of DES in this context. Acute Kidney Injury (AKI) and re-admissions are two distinct contributors to additional operational costs associated with endovascular procedures. Contrast administration during endovascular diagnostic or therapeutic procedures is one of the major causes of AKI and the third leading cause of hospital-acquired renal failure in the US. Over 50% of these cases are the result of contrast exposure during cardiac catheterization; this number does not include post-contrast AKI following angiography or endovascular interventions for peripheral artery disease. Contemporary evidence suggests that for patients with triple vessel disease, coronary artery bypass grafting is preferred over percutaneous coronary intervention due to better long-term outcomes, including lower rates of death, myocardial infarction, and target vessel revascularization. Similarly, for patients with left main coronary artery disease, both percutaneous coronary intervention and coronary artery bypass grafting can be considered, as they have shown similar efficacy in terms of major adverse cardiac events, but there may be a slightly higher risk of death with percutaneous coronary intervention. For proximal left anterior descending artery disease, both percutaneous coronary intervention and coronary artery bypass grafting are viable options, but coronary artery bypass grafting has shown lower rates of repeat revascularization and better relief from angina
