In the excellent video short "Two Minutes: What's the Risk? Tracking and Follow-Up" from ProAssurance, a critical notion was mentioned: the need to honestly assess our internal processes rather than merely relying on what's written in office policies. This concept is particularly resonant in office-based practices, where the standard procedure for tracking blood samples and tissue specimens sent for analysis is occasionally at odds with actual practice. While completing risk assessments for office-based clinics, I've observed that, although policies for specimen tracking often exist, the reality in some clinics is incomplete logs without regular follow-up. This discrepancy isn't just a clerical issue; it can lead to devastating delays in diagnosis. Physicians are often surprised when confronted with this deviation from the standard of care despite having a policy in place. It's a stark reminder that having a policy in place and staff understanding its criticality are two different things. "Work as prescribed" represents the ideal scenario—how tasks should be performed according to set rules and procedures. Conversely, "work as done" is an often imperfect reality. While some deviation is expected due to experience and clinical judgment, a significant gap between these can lead to oversights. It is important to acknowledge the need for a certain degree of flexibility; however, when it concerns critical elements of safe patient care, the discrepancy between how work is prescribed and how it's actually done needs to be as narrow as possible. We must continually audit, evaluate, and adapt our processes, ensuring that what's prescribed in our policies and procedures remains relevant in practice and doesn't just stay on paper but is effectively integrated into daily routines and practice workflows. #OralSurgery #PracticeManagement #PatientSafety
