We all want to recommend tests and treatments for our patients that work. But how do we know which tests and treatments truly help our patients? If I were teaching at an #optometry school, I'd make this book required reading for all students. Even though my match was over a decade ago, I still remember it like it was yesterday. Since I applied to urology, I actually matched early, which was an amazing gift. In theory, I had even longer to prepare for my cross-country move from Pittsburgh to Los Angeles. But in reality, I was really unprepared for my transition from medical student to physician. Looking back, there were a number of things that I wish I had done differently or better. Drawing on my experience, here are five things all medical students should do after matching into residency. To the Esteemed Medical Fraternity: We acknowledge the demanding schedules of doctors, often attending to 45 to 60 patients or more each day. However, it's crucial to recognize that post-operative care is a critical phase for patients. They experience pain, loss of appetite, and various other issues that need medical attention. Recently, a friend of mine underwent cardiac surgery. Despite the surgery's success, he struggled to meet his doctor for post-operative care as the doctor was occupied with new surgeries. This left my friend in a predicament, seeking out other cardiologists for follow-up care. My suggestion is to ensure that at least a nursing assistant or an intern with basic knowledge of managing post-operative pain and issues is available. They could take note of the patient's concerns, discuss them with the doctor, and provide timely feedback to the patient. This approach would prevent patients and their families from the distress of searching for new doctors during a vulnerable time. Ensuring continuous care and support is essential for a patient's recovery and peace of mind. Let's work together to enhance the post-operative care experience for all patients.

📃Scientific paper: Drivers of Scope of Practice in Family Medicine: A Conceptual Model Abstract: PURPOSE: The purpose of this study was to explore family medicine graduates’ attitudes and perspectives on modifiable and unmodifiable factors that influenced their scope of practice and career choices. By understanding how these factors intersect to influence desired and actual scope of practice decisions, we hope to inform strategies to address training and health care workforce needs. METHODS: During 5 focus group discussions, comprised of a total of 32 family physicians who either resided in or attended a residency program in western North Carolina, we explored family physicians’ attitudes and perspectives on their desired and actual scope of practice. We used thematic analysis to identify patterns in the qualitative data. RESULTS: We created a conceptual framework to understand the complex factors which influence family physicians’ scope of practice. Personal factors were found to impact desired scope, while workplace, environmental, and population factors influenced actual scope of practice. Stressors in each of these 4 categories often caused family physicians to narrow their scope of practice. Our study highlights specific supports that, if in place, enable physicians to maintain their desired broad scope of practice. CONCLUSIONS: Our study indicates that the national trend toward family physicians narrowing their scope of practice can be addressed by providing specific supports during training, residency, and mid-career. Quadratus Lumborum pathology I think about the QL as the opposite muscle to the middle scalene. The QL attaches the 12th rib to the lumbar spine and pelvis. The middle scalene attaches the 1st rib to the cervical spine. If the QL gets tight, it pulls down the entire rib cage. This puts tension on the middle scalene then it gets tight. A depressed rib cage makes it very easy for the middle scalene to deviate the 1st rib up and out of place. This puts tension on the stellate ganglion, phrenic nerves, brachial plexus, the sympathetic chain, the thoracic outlet, and causes widespread sympathetic autonomic hyperactivity, the leading, underlying cause of death and impairment. Because the middle scalene attaches to C2, when tight, it deviates C1/2, stimulating the trigeminocervical nucleus at C2, aggravating the trigeminocervical complex and furthering dysregulation of the ANS. This is all bad, but all totally fixable! Seeing a woman doctor might save your life. Women admitted to hospital in the US who are treated by female doctors are less likely to die or be readmitted to hospital, a new study has found. Meanwhile, the gender of a doctor for male patients admitted to hospital had no effect on their risk of death or hospital readmission. One Canadian study from 2021 found that female patients developed fewer complications when their surgeon was also female. Last year, another study from Sweden found that both male and female patients developed fewer complications and had shorter hospital stays when they were operated on by female surgeons.
