To unlock the power of data in healthcare, organizations across the healthcare continuum must focus on leveraging the right data – complete, timely, and representative data – at the right time. To make the transition to value-based care less burdensome, you must prioritize workforce, data, and resources. Make sure to educate your team about why you are making the shift. It is also critical to have data from the point of care. I listened to a 2015 BMJ roundtable discussion on "How can we improve patient-centered care?" the focus was on the importance of person-centered care. - https://lnkd.in/eZkuwUzD They had a lot of great contributors, including patient experts and clinicians. It was obvious that everyone was passionate about making care as person-centered as possible. However, what this looks like in practice was still very elusive at the end of the discussion. How can we take Person Centred Coordinated Care (P3C) and turn it into an actionable project? Reflective notes from the BMJ roundtable discussion: “How can we get better at providing patient-centered care?” Are patients interested in having their primary care clinician involved in the community, outside of their clinics? Will patients be receptive to practices setting goals to address health issues that matter to the population that are outside the QOF indicators? Can patients accept a negotiation around access to clinics, such as a practice reducing session hours on certain days of the week or month to engage in community outreach or quality improvement projects? A question from one of the patient experts: How can I know how well I can be?  How do you think the clinician should respond to this question? - Other statements/ideas made that resonated with me: There should be a recogisable partnership of trust between patient, clinician, and system. Patients play a crucial role in identifying what works and what doesn’t in the healthcare system, especially in terms of their own care. They share a critical position to appraise the resources needed for their care. Healthcare is about relationships. We need to identify the barriers to these relationships and eliminate them. No one has life completely figured out, but if you need some help, the healthcare system should be part of the solution and not make the problem worse. Have a great weekend.

What an inspiring discussion at the Duke-Margolis Institute for Health Policy #healthpolicy conference today! It is very hard to synthesize all the great insights and learnings. But it struck me that one key theme (at least) was shared across panel discussions: the need to make #pragmatictrials (i.e., #clinicaltrials conducted in real-world clinical practice settings including #primarycare clinics) work. From evaluating which Centers for Medicare & Medicaid Services #healthcaredelivery pilots and models work and drive real measurable cost savings and improvements in #healthoutcomes, to figuring out how to scale #publichealth programs that address social determinants of health #SDOH and break down barriers to access, to tailoring effective #publichealth interventions to the populations who most need them, to confirming clinical benefits of drugs FDA approved through the #acceleratedapproval pathway and collecting evidence for #CMS's coverage with evidence determination #CED, to evaluating the best strategies for efficient healthcare program implementations, and so much more, pragmatic clinical trials can be an important piece of the puzzle and generate some of the key data needed to advance evidence-based policies. But, in too many cases we lack the needed data systems and IT infrastructure, workforce, leadership, political will, and economic incentives. Luckily, there is a growing number of models and success stories out there, and a tremendous momentum to come together to advance data-driven health policies that improve population health. Partnership vs. Participation. This is the key to Patient-centered care that leads to positive outcomes.

Who is in the center of healthcare? The patient. 👱♀️ 👱♂️ 👩⚕️ Patient empowerment is crucial for improving healthcare outcomes, efficiency, and satisfaction. A study by the World Health Organization (WHO) found that patients involved in healthcare decisions have better health outcomes, such as improved management of chronic diseases like diabetes. Empowered patients show an increase in medication adherence, leading to fewer hospitalizations and emergency room visits. 📈 Furthermore, a survey by the European Patients Forum (EPF) revealed that majority of empowered patients reported higher satisfaction with their care compared to the one of non-empowered patients. 💰 Economically, the Health Literacy Survey European Union (HLS-EU) indicated that low health literacy, which correlates with low patient empowerment, costs EU countries a billions of € annually due to increased hospitalizations and longer treatment durations. What does "authentic" Value-Based Care mean for patients (and clinicians)? I define “authentic” value-based care as the measured improvement of health AND experience outcomes in relationship to the cost associated with achieving those outcomes.[1] The featured report from Humana states that "Data shows value-based care leads to better prevention, quality, patient experience, and health outcomes compared to traditional fee-for-service care."
