🔍 #VirtualWards are gaining traction and delivering impressive results. University Hospitals Plymouth NHS Trust has reported an occupancy rate of over 80% for its virtual ward beds, with plans to expand further. This is a testament to the growing recognition of the value and effectiveness of virtual wards in modern healthcare. In line with this trend, Lenus Health has been making significant strides in the realm of virtual wards as part of our end to end pathways. Our 12-week feasibility study demonstrates the incredible impact of virtual wards in ensuring patients with #COPD in remote areas receive the right care at the right time, all from the comfort of their homes through an effective early supported discharge (ESD) pathway. Having spent several years in the airline industry, we used to equate things to something that resonated with team for ease of communication. For example, depending upon the seat configuration, 500 people a day would fill about two Boeing 787s. It begs the question, why is this an acceptable number that equates to over 180k per year. Message me if you want to discuss how to help support organizations promoting patient safety and transparency. #medicalerrors #DFWBGH #healthcare #leapday https://lnkd.in/gCYMx7eN
#LeapDay happens just once every four years. But every day over 500 people die from preventable #medicalerrors in U.S. hospitals. DFW BUSINESS GROUP ON HEALTH is a proud Regional Leader of The Leapfrog Group. Join #DFWBGH and #Leapfrog to make giant leaps for #patientsafety through transparency. More than 2,100 #hospitals voluntarily report to #Leapfrog annually about the quality and safety of care they provide. Hospital at-home or acute #healthcare at home has been one of the leading trends in care delivery over the past few years. This innovative approach allows healthcare systems to expand their capacities without investing millions in building new brick-and-mortar #hospitals. Some of the nation's top hospitals and healthcare systems are leading the way, pioneering this transformative approach. The results so far have been remarkable, showcasing this model's effectiveness and patient-centric nature. What's particularly intriguing is the temporary waiver that has facilitated this shift. There's a growing sentiment in the industry that this may not be a short-term solution but could become a permanent regulation. As we approach the end of 2024, the discussions around making this waiver a permanent change are gaining momentum. Permanently instating the Hospital-at-Home care model opens up new possibilities for enhancing patient experience, reducing healthcare costs, and providing more personalized and compassionate care. #hospitalathome #clinicalinformatics #Caredelivery #healthcaredelivery #healthcareinnovation #valuebasedcare Link: https://lnkd.in/eVtk5bj9

UHUK members are leading the way through their innovative services 👥 Virtual wards and remote monitoring allow patients to get the care they need at home safely and conveniently, rather than being in hospital – and our members are providing this service across the country. 400 local patients have been cared for by FCMS (NW) Ltd through specialist virtual wards and remote monitoring, using state of the art technology. Without this service these patients would need to be cared for in hospital, but through the program 95% have avoided hospital admission. Virtual wards are an innovation that became vital in supporting Covid-19 patients through the pandemic and are example of the sector’s ability to respond to health challenges at pace and scale. Our staff are always trying to find ways to #improve the #experience patients receive every time they come through our doors. Teams of #clinicians in a number of areas across the service identified a few areas where #flow could be #enhanced, allowing us to do just that. This work was recently #recognised following Bendigo Health’s participation in the Department of Health Timely Excellent Care Collaborative (TECC). The health service was one of 14 #public #hospitals that took part and the #collaborative ran from January 2023 to June 2024. In that time, 12 change ideas relating to our theory of change were tested through the application of Plan-Do Act-Study (PDAS) cycles within three workgroup streams – Emergency Department (ED), Inpatient services and Hospital-wide Operations (Access and Demand). Bendigo Health received a Certificate of #Recognition and #Achievement for “achieving significant and sustained improvement as part of the Timely Emergency Care Collaborative”. #TECC #award #achievement #health #loddonmallee #hospital #patientflow

"Acute care hospital at home has grown from an experiment during the pandemic to a clinical model that many health systems have come to rely on to ease hospital capacity constraints. [...] Atrium has taken care of more than 9,400 patients since inception in March of 2020 and has saved greater than 33,000 brick-and-mortar bed days for its patients. The average length of stay is around 4.6 days. “From a quality perspective, our readmissions rate is 0.8, which is better than our brick-and-mortar facilities. We've had a mortality rate of less than 1 percent and our return to brick and mortar escalation rate is 5 percent. Currently, we do all this with very good patient experience,” he said. One of the reasons the program has succeeded is the ongoing acute care facility capacity constraint. [...] Our acute care facilities are under significant capacity constraints., so we have that as a primary driving factor for us to maintain our hospital-at-home program but also to drive the growth of our program,” Davis said. [...] “We are moving toward doing that for Car-T therapy as well. Those are outpatient, but they're receiving hospital-at-home level care in a modified fashion that allows us to be able to take care of them."" and link to the Go Home and Go Big: Strategies for Scaling Hospital at Home Programs Webinar https://lnkd.in/g-YUW4dE https://lnkd.in/gHuC_KzH
