I want to be in an ordinary atmosphere because I think that helps me.” John on getting treatment at home with nurse Annie through the Integrated Care for Older People (iCOPE) programme, as we continue to focus on our Healthy Lives Partnership week. iCOPE focuses on proactive care of people aged 65 and over. Livewell Southwest is working with Primary Care Networks and voluntary sector partners to identify frailty and support patients’ wishes for their ongoing care. Each GP practice holds regular meetings where therapists, social workers, GPs and frailty practitioners discuss the needs of patients such as medication reviews, social support networks and advanced care planning. This is helping reduce the amount of time older people are spending in hospital along with improving their quality of life and life expectancy. #HealthyLivesPartnership #IntegratedCare

🧓👵👴Do hospitals have a responsibility to train family caregivers? The article discusses a new law requiring hospitals to train family caregivers on how to care for patients at home after they are discharged. The law was inspired by cases like Iraq war veteran Doug Mercer who was left broken and brain-damaged after a motorcycle accident. His wife, Michelle, became his caregiver but struggled as she wasn't given any instructions on how to transport him safely. Family caregivers play an increasingly important role in caring for elderly or sick loved ones, but they often lack the training and support they need. The new law aims to address this issue by requiring hospitals to provide training on topics such as medication management, wound care, and how to use medical equipment.
