REALISTIC MEDICINE: TAKING CARE - Can design approaches help? Last week Sir Gregor Smith, Chief Medical Officer for Scotland launched his annual report ‘REALISTIC MEDICINE: TAKING CARE’. This report: ‘challenges our concept of how we think of care, and urges us to resist transactional, industrialised care so that we see each other in full fidelity, biology and biography' Just before the pandemic I co-wrote a paper for the Design Management Institute's Design Management Review with NHS Tayside, ENT Consultant Rodney Mountain. This challenged healthcare’s existing quality improvement paradigm, which is largely focused on concepts of quality improvement that have their origins in engineering and production control. We called the paper ‘DESIGNING REALISTIC HEALTHCARE IMPROVEMENT’ and described how design can disrupt and augment the current QI paradigm by: -              Putting the patient at the centre of our thinking -              Helping healthcare teams to see the bigger system -              Promoting creativity & innovation -              Building empathy -              Facilitating collaborative thinking If you are interested in how your healthcare team can upskill in design to help deliver on the REALISTIC MEDICINE agenda we have attached a copy of the paper. Exciting news! 🎉 Our latest paper, titled "Visibility-based layout of a hospital unit – An optimization approach", is now available in Health Care Management Science. I'm deeply thankful for the collaboration and support of my PhD advisor and co-author of the paper, Dr. Pratik Parikh. Thrilled to contribute to the realm of healthcare! Our study introduces a novel approach to optimize hospital unit layouts, designed to make it easier for nurses to keep an eye on patients. Through strategic placement of layout components, our model seeks to improve patient visibility, a crucial factor in reducing adverse events such as patient falls. The findings suggest significant improvements in visibility among patients, potentially leading to enhanced patient monitoring and safer healthcare environments. Healthcare has been a common thread in the academic career of Naomi van der Linden. "I use my research to assess how technology can generate value in the medical world, and quantify that value for decision-makers. Ultimately, this will allow us to prioritise interventions that generate maximum benefits for patients and the healthcare system as a whole. An important aspect here is to assess technology and policy not only on healthcare outcomes and costs, but also on the basis of their environmental impact, for example. And above all, I want to look at how innovations are affecting our demand for human resources, including nurses. There is currently a huge shortage of personnel, and they did not enter the healthcare sector just to push buttons. Like me, my health ambassador Irene Grossmann, MD, PhD is very interested in the acute healthcare system. That system is undergoing many changes, including the aim to concentrate high-complex care in a limited number of locations, while organising low-complex care to be based closer to patients' homes. At the same time, the ageing population is having a major impact, with more patients with more complex problems being admitted to emergency departments. How do you make sure all these people get to the most appropriate place, which is by no means always the hospital? Mayo Clinic's Journey with Design Thinking in Health Care 🏥 🚀 The Challenge of Transformative Change: Mayo Clinic's journey began with a pivotal question from physician Nicholas LaRusso in the early 2000s: Could new kinds of doctor-patient interactions be tested, similar to how new drugs are tested in clinical trials? 💭 Thinking Beyond Tradition: Recognizing that while medical advances were significant, healthcare delivery systems remained largely unchanged, Mayo Clinic sought inspiration from design firms like IDEO. LaRusso and Dr. Michael Brennan initiated a skunkworks outpatient lab, SPARC, to experiment with innovative ways of provider-patient interactions. 🌱 Cultivating Transformative Ideas: SPARC faced challenges, from recruiting physicians for untested research to bridging the cultural gap between physicians and designers. Despite hurdles, the lab evolved into the Center for Innovation (CFI), an enterprise-wide research institute dedicated to studying healthcare provision processes comprehensively. 🎯 Pursuit of Transformational Change: By 2010, the CFI had become a respected consultancy within Mayo Clinic, undertaking projects in service redesign, patient education, technology integration, and exam room redesign. While acknowledging the significance of their innovations, the team aimed for "transformational" change in healthcare delivery. 🔍 Key Questions: As Mayo Clinic's CFI grew, questions arose about achieving transformative goals. What would a major change in healthcare look like? How should the impact be measured? Were the center's structure and processes suitable for transformation? 🗒 Conclusion: Mayo Clinic's journey exemplifies the power of design thinking in healthcare, pushing boundaries, and seeking transformative change. Amid ongoing headwinds facing the health care sector, Lifespan health system and Brown University expanded their affiliation to strengthen top-quality patient care, medical education and biomedical research in Rhode Island.   Lifespan will change its name to Brown University Health — to be referred to commonly as Brown Health — later this year through a rebranding effort, enhancing its ability to recruit and retain world-class physicians and reflecting a deeper alignment between Lifespan’s clinical care and Brown’s academic and research focus. The affiliation also includes reciprocal financial investments between Lifespan and Brown, which will continue as separate, independent organizations after the implementation of the Lifespan rebrand. In most of life, we know the quality of the things we buy - both products (shoes, cars) and services (haircuts, cleaning). But when it comes to healthcare, it's extremely difficult for consumers to discern quality. That was the point Fawwaz Haq, MD, MBA drove home in my class. Fawwaz is an internist and a health equity researcher. He serves as a technology strategy director at Elevance Health. I asked him to come speak about the consumer experience.   Health systems market the same things. They all boast of expert doctors and nurses, modern equipment, and updated facilities. So whom do you believe? Going in for a procedure, how do you know you have a skilled surgeon? One student reflected on the difficulties of this. She is a pediatric intensivist and executive medical director of quality. She went through the options:   - There is Healthgrades but not many people know of it. - There is Yelp but reviews heavily skew great or terrible. - There are "Best Doctor" awards but those are paid ads.   For institutions, there are rankings like US News. But those rely more on reputation than people assume. One colleague in academic research says everybody hates rankings until they do well.   Indeed, when they rank highly, they broadcast it to the world: "This is validation of our caliber by an objective third party." The next year when they drop a few spots, suddenly "the algorithms are flawed and biased." Well, which is it? You can't have it both ways. In other cases, it's a pure marketing play. I lived in San Diego County for 10 years. Every year, UC San Diego Health advertises their #1 in San Diego ranking. On the one hand, it's a fine institution. I have great respect for the physicians there. But on the other hand, it's comical. #1 again, huh? Good for you. As US News rankings go, UCSD literally has zero competition. It's the only academic medical center around. No other hospital is eligible in enough categories to earn that composite score. Recently, this "best hospital" in San Diego acquired the worst hospital in San Diego. We'll see if their quality carries over. I'll refrain from making predictions. But this raises another important point: Healthcare brands further obfuscate the issue. Just because an institution is renowned doesn't mean all its service lines are excellent. And just because a program is reputable doesn't mean every surgeon is experienced. From a marketing lens, health systems can do many things to build brand awareness, educate the public, and attract patients. Clinical outcomes data are important. Stories are powerful. And as Fawwaz emphasized, physician relationships are key. Care is directed by the PCP and referring specialists. But from a patient lens, finding quality is tricky. Your choice is guided by your doctor, informed by your neighbors, and limited by your insurer. You're left to make the right call based on - or perhaps despite - the marketing you encounter. #healthadministration #healthcarequality #healthtech
