Happy to share our first publication of 2024! It is now well-established that complete surgical resection of MEG dipole clusters, reconstructed from interictal epileptiform discharges, is associated with favorable seizure outcomes. However, the impact of MEG cluster resection on the surgical resection volume is not known, nor is it clear whether this association is direct and causal, or it is mediated by other predictive factors. This information is essential for assessing the true clinical value and utility of MEG in epilepsy surgery, but was missing from the extant literature. In this paper, we have clarified these open questions and assessed the diagnostic accuracy of the MEG approach in our center. We found that complete resection of interictal MEG dipole clusters leads to good seizure outcomes in temporal as well as extratemporal lobe epilepsy without affecting the volume of surgical resection and independent of other prognostic factors, such as MRI finding, localized ictal EEG, EEG-MRI concordance and pathology. Our standard MEG approach can localize the epileptogenic zone with high accuracy (86%) and diagnostic odds ratio (42.5). These results provide crucial information, which was missing to date, supporting strong clinical value and utility of MEG in epilepsy surgery. We emphasize that MEG mapping should be used whenever feasible to improve postsurgical seizure outcomes and extend surgical treatment to patients who may not have been considered as candidates before. #epilepsy #meg #neurosurgery #neurophysiology

Attila Rácz, M.D., Ph.D., from the University Hospital Bonn in Germany, and colleagues evaluated long-term outcomes and potential influencing factors among 107 patients who underwent surgery for neuroglial tumors (2001 to 2020) at a single institution. The researchers found that 75% of the patients who underwent surgery achieved complete freedom from seizures at 12 months and 56% achieved complete freedom from seizures at the last follow-up visit (70.4 months; median: 40 months). For both 12-month follow-up outcomes and the longest available outcomes, completeness of resection was a crucial factor, while there was no consistent impact on postsurgical outcomes for lobar tumor localization, histology (ganglioglioma versus dysembryoplastic neuroepithelial tumor), history of bilateral tonic-clonic seizures prior to surgery, invasive diagnostics, side of surgery (dominant versus nondominant hemisphere), age at epilepsy onset, age at surgery, or epilepsy duration. Patients who underwent lesionectomy and lesionectomy with hippocampal resection showed similar outcomes. "#Neuroglialtumors present as excellent surgical substrates in treating structural epilepsy," the authors write. "To achieve an optimal postsurgical outcome, a complete lesion resection should be pursued whenever possible."

In fact,these #patients are less than half as likely as #strokepatients who haven't undergone #heartsurgery to receive what could be lifesaving and disability-preventing treatments, according to a Yale School of Medicine study recently published in JAMA Neurology #Endovascularthrombectomy, which involves #snaking a #thin #catheter #up an #artery from the #wrist or #groin to #clear #arterialblockages in the #brain,is by far the #most #effective #treatment for #ischemicstrokes,which are the #most #common #form of #stroke—#caused by a #blockage of #bloodflow to the #brain. However,the #treatment must be #administered in the #first #24hours of #symptoms, #ideally #sooner For a variety of reasons, detecting strokes in heart surgery patients this quickly can be tricky. Also,surgery patients are generally not eligible to receive the other major treatment modality for ischemic stroke,intravenous thrombolysis with tissue plasminogen activator (tPA),due to its blood-thinning effects To understand whether heart surgery patients are less likely to receive post-operative endovascular treatment if they suffer strokes,Adam de Havenon,MD,a vascular neurologist and associate professor of neurology at Yale School of Medicine,and his colleagues sifted through a nationwide database of stroke patient data from nearly 5,000 hospitals between 2016 and 2020 The team found that,following heart procedures,patients who suffered ischemic strokes within three days after their procedure were less than half as likely to receive endovascular stroke treatment as patients whose ischemic strokes did not follow cardiac treatments #Cardiacsurgery,such as valve repair or implanting a stent,carries a high risk of triggering a stroke by potentially causing a clot that goes to the brain or damaging the arteries that carry blood directly from the heart to the brain. The procedure is second only to brain surgery in terms of stroke risk following surgery. Cardiac implants,like pacemakers or stents,can also raise the risk of stroke following surgery "The data show that even up to 2020,we were not offering the single most effective treatment for ischemic stroke that we have to individuals after cardiac surgery," said de Havenon, who led the study. "There's a major disparity here." The researchers used the National Inpatient Sample database, a large database that captures data from inpatients at thousands of U.S. hospitals. The team analyzed data from more than 630,000 stroke patients, including more than 12,000 who'd had cardiac procedures.

A quantitative retrospective analysis was performed, based on a master's thesis, of 34 individuals with this pathology, taking into consideration: gender, age, life habits and type of treatment. These factors were associated with the projections of the number of new cases, starting in 2018, according to INCA and IARC data.
