Frustration mounts over repeated E.R. closures across B.C. interior Many a time during my career, I had to address problems with shoe-string budgets. This meant looking outside the box for cost effective solutions that didn't rely on multi-million dollar budgets. I had a boss tell me, "Learn to drive the Jalopy before you get the Cadillac". As a result of having to find cost effective, creative solutions, it becomes ingrained in your mind. Every year we graduate a few hundred new doctors between UBC and UHNBC here in British Columbia. When we look at these small hospitals, like Merrrit, Chetwynd, Mackenzie, and Haida Gwaii, these hospitals are designed to handle the basic ER activities. More major cases are usually transferred to a larger center with a specialist. So here's the thought. Why aren't we allowing our final year medical interns to cover ER's in smaller centers? The interns would still have access to supervising doctors in larger centers which they could converse to via, skype, phone, text, cell etc if they require additional help. You could also send 2-3 interns to these ER's. In exchange, the interns gain valuable experience and they could also receive renumeration or additional credits for their hours served. The province would pay for the transport the interns to the ER's. This could be personal vehicle, charter flight for more remote locations of medivac backhauls. How do you describe your role in the NHS? The term "healthcare scientist" has been in use since around the 1990s as a collective term for our many and varied professions. Some of the challenges with the (admittedly difficult) task of bringing education routes and career paths together have left a range of feelings about the term. And in around 2003, Dr Owen Crawley brought first radiographers and then operating department practitioners under the professional leadership of the Chief Scientific Adviser for Wales too. Skip to 2024 and we have over 7000 healthcare science professionals in NHS Wales including over 70 consultants! But still there are many different views on identity and what we call ourselves. Perhaps it is no wonder that people on the street still talk only of doctors and nurses. We are keen to hear from you... What do you consider your identity to be? Is this different to how your friends and family describe your role? If you are a Welsh speaker, do translations of these terms reflect your identity? In 2023, I decided to take on a project close to my heart and work with A/Prof Matthew McGrail (The University of Queensland) to evaluate the Remote Vocational Training Scheme (RVTS) model. I'd been long interested to understand how well it worked and why including to inform World Health Organization and member states of relevant rural health initiatives. The doctors the RVTS supports are already employed in rural and remote practices and Aboriginal Medical Services (average 1.6 years) when they join the program. RVTS staff remotely supervise and tailor support for them (phone, computer and educational webinars) to train as a GP or rural generalist whilst they remain in the same practice for 3-4 years (with two offsite face-to-face workshops per year). It's required because of supervisor shortages in more remote settings. Well, what did we find? Great results. And some key learnings. https://lnkd.in/g44HBP5v It’s a well designed and implemented program, accredited and aligned with Australia's two GP colleges Australian College of Rural and Remote Medicine (ACRRM) and The Royal Australian College of General Practitioners (RACGP). It achieves strong continuity and retention outcomes, especially for challenging environments with mostly international medical graduates who are commonly working in these locations and who have nuanced needs. Realist evaluation showed the program fosters a sense of comfort, confidence, competence, belonging, and bonding which drives the good results – telling us that sustaining someone in fellowship training in resource challenged settings is much more than teaching GP skills. Being in once place, the RVTS doctors also built trust with patients, established new models of care and contributed to sustainable primary healthcare teams. So, what does it mean for rural policy? Getting more rural and remote doctors into models where they can train continuously in one location/region, holistically supervised and supported, can work well. And it is great fun working with the data and the people from a 24-year-old rural program. But you might need to read the supplement to find out more.
