The Growing Challenge of Workplace Violence Against Nurses: Q&A with UPMC Chief Nursing Officer Maribeth McLaughlin As UPMC and other systems in the state and across the country continue to navigate the complexities in health care, one critical issue that needs immediate attention is the escalating incidence of violence against nurses and other health care workers. This is an issue that not only impacts those providing patient care but also the patients themselves.    I recently answered some common questions on this topic, including why I think instances of workplace violence have increased and how UPMC is working to address that increase through creating a culture of safety.   We must work together to ensure that every health care worker is able to do their job without fear of harm. Eighty percent of trafficked persons have reported going to a healthcare provider while in captivity. As healthcare providers, we must report the abuse of minors and support those dangers. If you are in Texas, you are required to take human trafficking training every 2 years. Several other states, including Michigan, also have a training requirement. Take just a couple of hours of your time and learn the clinical signs, know how to screen, and how to follow through with a report. A small amount of your time and attention could equate to saving someone’s life. A recent study reviews the care provided by Sexual Assault Nurse Examiners (SANEs) to female sexual assault survivors at 13 US emergency care centers and community-based programs. The findings are remarkable: survivors consistently reported receiving high-quality, compassionate care. This includes feeling their needs and concerns were taken seriously, not being blamed for the assault, experiencing genuine care and compassion, and being clearly informed about the examination and follow-up procedures. What's even more significant is that these positive perceptions did not vary based on the patients’ demographic characteristics or their health status before the assault. This consistency underscores the dedication and professionalism of SANEs in providing equitable and empathetic care to all survivors, regardless of their background. This study highlights the critical role SANEs play in supporting sexual assault survivors and ensuring they receive the respect and care they deserve. Conflict resolution, communication and de-escalation must be educational standards for nursing and all healthcare staff! They are skills that will define your career and are more important today than ever! Had the honor of being part of the Bates Technical College MA advisory board yesterday. I brought up the need for additional training regarding communication and conflict resolution. Conflict, poor communication and incivility are a major threat to our workforce. It picks away at caregivers sense of connection, community and purpose. All of those are parts of individual resilience. So, to solve for engagement and retention--you need to train and teach conflict resolution. (AND address violence and patient code of standards) I coach more staff for dealing with conflict than any technical aspect of the job. I bet if you asked most nurses--would you rather deal with an irate patient/family member or a patient in full arrest. Most would answer the code--that is because we are trained and have a systematic approach to asystole--not conflict. Not to mention we have a team approach to a code and not a conflict. Conflict resolution and de-escalating communication need to be taught throughout nursing school and into clinical care. Tactical empathy--a technique taught by Chris Voss should be a universal practice for all healthcare staff. That’s what makes workplace violence toward nurses so troubling — it strikes at the heart of their mission. According to the U.S. Bureau of Labor Statistics, health care workers are five times more likely to experience workplace violence compared to workers in other sectors. No one should have to accept violence as a part of their job. At least in East Tennessee, the psychiatric facilities are moving toward minimal medical staff with nurse practitioners providing almost all of the care, even brand new graduates! I am married to a psychiatric nurse practitioner who is excellent and worked as an RN for 20+ years, but many of the current NPs and also Physicians’ Assistants are functioning as attending psychiatrists. This is a dangerous practice! Joint Commission expects a hospital to have a medical staff that oversees the clinical care at a hospital. However, coupled with this trend, psychiatric facilities are hiring inexperienced leadership as CEOs, who don’t understand how a hospital should run, continue to allow and demand greater responsibility to the NPs with less oversight from MDs. To become a psychiatrist there is 4 years of college, 4 years of medical school, and 4 years of residency, just like any specializing MD. Compare that with becoming an NP with prescription rights, which can occur as quickly as 2-4 years.
