Great conversation happening now about rural communities and the challenges we face around healthcare. Cancer care and access to that care is a large part of this. This work is hard, impacts the whole community, and needs to involve cross-sectional work. Regulatory impact analysis (RIA) guidelines in over 30 countries require some kind of distributional analysis even for individual regulations. Distributional analysis to account for inequality across an array of categories is now required in the United States under the new revisions to the RIA guidelines. Yet no good methodology exists that can be applied at the level of individual regulations. Distributional analysis in ex ante RIA in essence tries to look at the macro effects of micro interventions. Basically, since no practical methodology exists to assess final distribution of costs and benefits of micro interventions, regulators in every country are faking the analysis today. Some interesting examples provide some channels for further work. Canada carries out a kind of a suitability analysis looking at the effect of the preferred option on vulnerable groups such as the elderly and the disabled and that has proven to be useful such as in the case of regulating plastic straws. I very much hope that the work of the Society for Benefit-Cost analysis can contribute to the emergence of a practical and policy relevant methodology that can be included in ex ante regulatory analysis.
