A significant proportion of the UK - 30% - have one or more long-term conditions, many of which are complex, and aren't being treated 'to target', With such a burden of care, how can we identify those most at-risk, ensure they see the right practitioner, and receive personalised treatment? Dr Shona Schofield, an NHS GP working at Ball Tree Surgery in Lancing, has been using the Metadvice software platform. It enables her team to prioritise patients by risk (not date of birth), such as those who would benefit most from a medication change. Patients are then allocated to the most appropriate clinician where they receive personalised recommendations, based on their medical history and the latest guidelines - which are displayed together on one screen. One of the most difficult tasks in the direct-contracting space is to help patients understand that the care given in ambulatory surgery centers is second to none. So many patients think that a routine or elective knee scope or hernia surgery needs to be done in the "Big Hospital" because that's where the doctor wants to do it. Instead, those kinds of common surgeries are best served in an ASC when appropriate for the patient. In a recently published article, our collaborate team from New England Baptist Hospital, Brigham and Women's Hospital, and Beth Israel Deaconess Medical Center discuss the costs associated with nonoperative versus operative management of meniscal tears in the 2 years after diagnosis. In our study from the MarketScan databases that included included 29,924 patients, the mean cost of management per patient was $3835. Costs were lower in the Non-Surgical group ($1905 ) compared with the Early Surgical group ($6759), while the highest costs were observed in the Late Surgical group ($7649). In summary, nonoperative management had the lowest cost burden and should be recommended for patients with appropriate indications.
