Many of the young people we support have experienced significant childhood trauma and face numerous challenges, including unstable home environments, abuse, neglect, and abandonment. The Connections to Care (CTC) program provides comprehensive, coordinated, and evidenced-based services for individuals and families with a serious mental illness (SMI), serious emotional disturbance (SED), or co-occurring disorder (COD) who are also experiencing homelessness and are at imminent risk of homelessness. Services take place where individuals experiencing homelessness spend time—on the street, in the subway system, in a drop-in center, or shelter. This is a uniquely community-based program where 80% of the work takes place right where clients live/spend time. This program focuses on individuals over the age of 55 who have had a difficult time accessing behavioral and primary healthcare.
