It can be difficult to maintain your autonomy as a physician in medicine. Decades ago, you could have a very stable practice, even a solo practice, and still make it work. If a doc tries to start a solo practice today, they will be underwater before they know it. Why? It used to be that billing and collecting from health insurance was much easier. Insurance companies would generally pay, and reimbursements adjusted for inflation were higher. You also needed a whole lot less staff to run a practice as health insurance companies weren't playing the games they are now. Currently, is quite the opposite. Reimbursements continue to go down, and the last thing a health insurance company wants to do is pay you for your work. They get away with this by making a doc have to jump through hoop after hoop; the doctor needs to hire hoop jumpers. These hoop jumpers are expensive and combined with the low reimbursements it squeezes small practices out of business. We are even seeing this now with very large specialty practices, despite the insanely high volume of patients they are seeing. The irony of the whole situation is that a health insurance practices are very busy and insolvent at the same time. Direct care anyone? #healthcare #medicine #directcare
