I talk to A LOT of patients/brokers/employers about Direct Primary Care. Many times, the reaction I get is -"This sounds great, why isn't everyone doing it?" (I ask myself this question ALL THE TIME) 😀 😉 Here are some of the challenges/concerns I hear from prospective patients and I rebut each in my latest podcast- Full Scope with Dr Nicole Hemkes. 1."I already have insurance" DPC is not insurance. Insurance doesn't mean you have accessible or affordable healthcare. 2."I don't have the money for DPC." 💰 This is not Concierge Medicine. Our DPC is an average of $70/month. 3. "This sounds too good to be true. There must be some catch." Come in and see for yourself. 4. "I don't really go to the doctor, why would I join DPC?" You are paying for access, not utilization. One visit to an urgent care/ER can cost a lot more than ONE YEAR of Direct Primary Care. 🏥 5. "My employer pays for my insurance benefit, so why would I pay this additional cost?" Quality, time, access, doctor-patient relationship, lower out-of-pocket costs 6. "I don't see anything wrong with the current healthcare system/health insurance." Please considering sending to your local reps and pass this along to as many as possible, we need to fix prior authorizations for Medicare plans. Prior auths are absolute NIGHTMARES! I had a patient just a week ago, had every insulin excect fiasp and novolin N be denied because those were the plan's preferred Medicare OPTIONS. While processing the prior auth , they didn't post what was preferred until after it was denied after trying to submit a few types, 1. already delaying care. 2. There was a national shortage of fiasp and the patient needed fast or rapid acting insulin, NOVOLIN N is NOT even an equivalent or the same insulin class. The prior auth was then denied. 3. I tried to call for an urgent appeal and they could care less, they told me an urgent appeal needs to be submitted via fax and still takes up to 3 business days. It took all I had professionally on that phone to hold my composure. I had to write a professional but very nasty letter to APPEAL their denial decision stating they were going to kill the patient by denying her something equivalent or similar to fiasp, when there's a national shortage and how NOVOLIN N isn't in the same class. This delayed patient care and jeopardized her safety as her blood sugars were dangerously high. If she was going to go into HHS or DKA, become hospitalized or even caused DEATH it would have been the plan's negligence with their preferred insulin choices, PA denial decision and significant delay of care due to their processes- - and yes you betcha I put that in that appeal letter that it would have been their fault. I shouldn't have to write appeals and prior auths for an insurance plan's negligence... Ironically after sending that letter and saying it would be their fault if something happened, 15 minutes later it was miraculously approved. This delayed getting her insulin for 3-4 days. SOMETHING NEEDS TO BE DONE

Deja Vu... Almost every health system that has merged with others to form dominant market shares claims that they "can offer higher quality at lower total cost." In general, this has been a deliberate misrepresentation, as most research has shown that quality does not improve and in some cases worsens, while costs, on average, have gone up significantly (6%-18%) "According to KFF, hospital mergers and acquisitions can lead to higher prices, but the evidence on quality is unclear. Some studies indicate that cross-market mergers can increase healthcare prices, and research from Elevance Health found that independent hospitals that are acquired by health systems have higher prices and lower quality of care. However, other research shows that hospital mergers can reduce operating costs by 15–30%, but these cost reductions don't translate into lower prices. Instead, research shows that hospital mergers increase the average price of hospital services by 6–18%". Novant, in its defense against the Federal Trade Commission suit to block its acquisition of Lake Norman Regional MC, asserts that it "can offer higher quality at lower total cost." Yet it's been 3 years since Novant acquired New Hanover Regional Medical Center in Wilmington, NC (the only hospital in town), and most metrics suggest quality is below average. The hospital received 2 stars from CMS for 2 years in a row (out of 5 stars) and state and federal monitors found the hospital out of compliance with Medicare COPs in 2022. While it received a "B" grade from The Leapfrog Group, when you look at each individual metric in the report card, the scores look pretty average or even below average. Now, a former patient is speaking out and he happens to be a physician with expertise in hospital quality and safety.
