I am thrilled to be attending PRECEDE Fifth Annual Meeting in New York — a summit gathering the world's foremost experts dedicated to the mission of enhancing early detection, screening, risk assessment, and prevention for individuals with a hereditary risk of pancreatic cancer. This consortium operates through a pioneering model of open collaboration and data sharing. The Pancreatic Cancer Early Detection (PRECEDE) Consortium represents an international, multi-institutional collaborative effort among experts aimed at improving survival rates for pancreatic cancer patients. Its primary objective is to transform early detection and prevention methods for pancreatic cancer, aiming to increase the current 5-year survival rate from 10% to 50% within the next decade. With a presence in over 30 leading academic medical centers worldwide, PRECEDE has amassed the largest high-risk patient cohort, incorporating longitudinal clinical data alongside biospecimen acquisition and tracking. Here is a good example of how Therapyte used NLP and LLMs to process Real World Data from Electronic Health Records and proceed from unstructured to structured medical data, to document therapy regiments and extract population insights in a cohort of breast cancer patients.

Sacituzumab govitecan, an antibody-drug conjugate (ADC) approved for locally advanced or metastatic urothelial cancer (mUC), triple-negative breast cancer, and recently for HR(+) HER2(-) breast cancer, offers a promising treatment option. While patients may experience side effects like neutropenia, diarrhea, and nausea/vomiting, proactive adverse event management can help patients stay on track with their therapy and achieve positive outcomes. This study changed practice for patients with advanced melanoma when it was first reported in the NEJM in 2015, showing the immunotherapy drug ‘pembrolizumab’ prolonged survival with less side effects than the previous standard of care. The most recent update, reported in the high-profile journal JCO has confirmed that pembrolizumab provides long-term survival benefit in advanced melanoma, giving additional hope to patients who respond to this treatment. Real-world datasets come to the rescue here, providing insights into treatment possibilities among diverse female patient populations, including those with concomitant diseases. Unlike randomized clinical trials, they are not limited by inclusion/exclusion criteria, allowing us to evaluate drug combinations more comprehensively. Oncologic Drugs Advisory Committee (ODAC) meetings have been critical in obtaining independent expert advice on scientific, technical, and policy matters for cancer therapies. After many years of watching ODAC meetings, see evolution into truly deep diving and wider conversations. In the recent discussion titled "Conversations on Cancer: ODAC Chronicles - Past, Present, & Future of Oncology Advisory Committees" sponsored by Oncology Center for Excellence at FDA, several interesting observations were shared by outstanding panel of current and past ODAC members and chairs. In the past, all oncology marketing applications were discussed at ODAC, but this is no longer feasible considering the explosion of development in oncology over the last decades. Process is now focusing on cases where there is complex deliberation needed for the product benefit with even more emphasis on better handling of clinical benefit estimate. Proposal was shared to bring independent disease expert(s) into forum to move away from company presentation of the disease state. Find that very useful thought and a move toward getting better grasp on true clinical benefit, fit for practice and most importantly stronger patient-centric approach by hearing voices that emphasize with patients while dealing with medical challenges of beating cancer.
