What hospital services will never leave the #hospital? 🏥 👩⚕️ As a seasoned #obstetrics and #gynecology physician, I can tell you that while virtual and digital care have their place, there will always be a need for #inpatient care. Emergent situations in my specialty like breech in labor, ovarian torsion, ruptured ectopic, preeclampsia, preterm labor, and postpartum hemorrhage require physical presence and management that cannot be replaced by a robot or video visit. 👩💻 But that doesn't mean we can't bring new #technology into the hospital to improve the care delivery experience. As healthcare providers, we need to help tech innovators understand what we really do. Let's start by sharing the #alwaysinpatient list for our respective specialties. This weekend, hundreds of thousands of #physicians and #providers will be taking on-call 📟 duties at hospitals across the US. They will be away from their family and friends, caring for the urgent and emergent needs of their patients. Call coverage is typically additional work hours in excess of a regular and full workweek. Their willingness to be available is a testament to the #passion and #commitment they have to care for those in need. The question is, "Does Brand Name Equal Quality in Health Care?" Not to bury the lead....YES, but..... "We found that brand-name hospitals do in fact deliver higher-quality care than the national average, although patients who receive care at these hospitals don’t always get the best treatment.".... Keep Reading..... "In fact, the worst-performing doctors at brand-name hospitals deliver much worse care than the national average. Furthermore, patients receiving care at a brand-name hospital outside the main campus actually receive no better care than average, despite these facilities having significantly higher costs." That information is absolutely terrifying if you 1) aren't able to truly know if your doctor is actually a good doctor, and 2) if you are in a rural community where your community hospital has been acquired by a large hospital. It is really important for the American public to be able to have access to such information to make decisions that could save their lives... What a great article from Nick Reber at Garner Health!

The plight of rural hospitals in the US is an urgent and pressing issue that demands our immediate attention—more than 300 risk closures in the next two years. While hospitals cannot be everything to everyone, basic medical needs still need to be met in many rural communities. Whatever happened to the notion that healthcare is local? Rural hospitals have been struggling to recruit specialty physicians and surgeons for years, long before the pandemic. In addition, there is a national shortage of primary care physicians, and patients have few choices but to make repeat visits to the local ED. Bottlenecks in EDs and overburdened primary care practices are at a breaking point in many regions. As physicians exit clinical medicine, patient access is diminished. Patients go without care, are forced to accept delayed care or drive long distances to seek care. Patients often suffer without timely care, and the complications and costs rise as medical conditions go untreated. Once patients find care elsewhere, they often stay there for future care. Dwindling patient volume and decreased reimbursement is a death knell for rural hospitals and their communities. Rural hospitals are often the largest employer in a community. Today, one of the partnerships I celebrate is VirtuCare, a hybrid model providing urology clinical services to hospitals struggling to recruit surgeons. VirtuCare has experienced great success and is in year three in rural hospital systems, with many expanding services as awareness in the community increases. Daily clinics, where APPs perform physical exams and manage diagnostics, develop a care plan in collaboration with the urologist and the surgeons performing ambulatory and surgical procedures on-site monthly, making the future brighter for patients in need and rural hospitals. Daily conversations with hospital CEOs and their executive teams give me hope for better patient access. Relational medicine starts with trusted and respectful partnerships, and today, I see so much hope for physicians wanting a better practice model and hospitals needing innovative solutions to open patient access. I contributed, on behalf of Apha, to The Australian Financial Review piece which highlights the financial viability and sustainability challenges being experienced by the private hospitals sector. Private hospitals are about the services Australians need. Each year the sector treats 4.8 million patients and provides 32 percent of ICU separations, 61 percent of all surgical services, more than 80 percent of all in-patient rehabilitation separations, 45 percent of all acute adult psychiatric beds, 54 percent of all chemotherapy. Australia has long enjoyed and relied upon a balanced health system. A system where the public and private system combine to deliver the health care needs of the Australian community. Private hospitals are essential to the health system as a whole. The private sector provides more than 30 emergency departments, more than 100 intensive care units, 1.6 million surgical procedures each year, 1.6 million medical admissions, 70,000 admissions for childbirth, 1.4 million days of intensive multidisciplinary rehabilitation, 976,000 days of acute overnight psychiatric care and 240,000 days of ambulatory mental health care. APHA welcomes the Federal Health Minister’s recognition of the important role private hospitals play in Australia’s healthcare system. APHA has particularly welcomed three important announcements made by the Federal Government in the last couple of weeks; Firstly the Department of Health and Aged Care’s Private Hospital Sector Financial Health Check – APHA welcomes this important work crucial in directly informing the Federal Government’s response to the urgent issues raised by APHA. Secondly, the commitment to a two-year trial of MBS funded video in-patient consultations for psychiatrists, a specific measure called for by APHA in response to the crisis in access for in-patient care in psychiatry. Thirdly, the reversal of the decision to remove General Use Items from the Prescribed List of Medical Devices and Human Tissue Product meaning these products will remain on the list following 1 July 2024. It is clear urgent action is required to ensure the balanced healthcare system Australia has relied on is maintained. This country needs a strong and sustainable private hospital system to support and ensure the viability of a strong public health system.

Two thirds of all planned surgical procedures are undertaken in private hospitals and day procedure facilities. With unprecedented waiting lists for surgery in our public hospitals the health of the private sector underpins the health of millions of Australians.
