Interesting article looking at the challenges that face Primary Care and how we stack up against other nations. There is no one size fits all when it comes to medicine and no one country has it all figured out. From my perspective one of the biggest challenges I see is how uncoordinated our care is here in the US. Right hand very rarely knows what the left hand is doing and it is detrimental to our healthcare. There is a lot of talk of care coordination, but very little in practice. 7moWhen running an actual care coordination for 80+ year olds and their adult children, so many asked “Who was the quarterback driving the game?” While I know we would love our primary care to be partnered with families as I did as a nurse running a privately run care coordination business, I saw the hours it took to pull information together from home care, specialists, therapies, the patient and the family to present a summary to even help the PCP give insights for the best plan. I think there is huge opportunity to better coordinate care when we put the health data with the family But, we need to not just give them a dump of health data, but the tools to edit, collaborate with others, and organize their health stories so that they can share for better care. There is a long road ahead but everyday I am encouraged by what we are seeing at Primary Record in partnering with 1upHealth, Inc. to tap into the patient access and information blocking laws laid out in the 21st Century Cures Act. But it will take the advocates, nurses, and families demanding from our community into the system to create the needed adoption and change of today’s healthcare system!
