Clearly Bismarck Healthcare used in Germany, Switzerland, Japan and other democratic nations is the answer enabling access for all citizens to quality affordable private insured and private #ffs manufactured medical care with more private insurers and private hospitals per capita than in America and better clinical outcomes and costs than in the USA. As folks "try" to move to value based care the corporate robbers are moving faster to extract profits from what is supposed to be a care system, but has become a system of transactions, in which instead of transactions supporting success for care, care is now the excuse for billing the transactions. This is progressing along, and has for 45 years. Stopping it requires reinventing healthcare altogether: healthcare access should be a RIGHT, with everyone covered for the vast majority of care, and every community with adequate primary care, regardless of location. National consensus will require a grassroots movement of care providers and patients. It's past time.

Provider access challenges are real. Innovative primary care policies are transforming access to team-based care, providing new pathways for primary care providers (PCPs). "No One Can See You Now...." Another day, another headline about primary care in crisis. Of the reasons outlined as contributors to the crisis, what resonated with me was #3 "The US continues to underinvest in primary care...since 2019, primary care investment has steadily declined for all payers. This decline is most pronounced in the Medicare population...." Without addressing this, it will be challenging to address the rest. This neglect not only weakens our health system but also adversely affects the nation's health, evidenced by long wait times for primary care visits and a shrinking number of primary care physicians.
