Keratoconus is the most common corneal ectatic disorder worldwide. Currently, it accounts for nearly 30-40% of the corneal tertiary referrals, placing significant burden on the ophthalmic services. To address this issue, several innovative measures have been introduced, including the set up of virtual clinics and allied health professional (AHP)-led clinics. In our recently published article in the Eye journal, we highlight the effectiveness, safety and feasibility of AHP-led keratoconus service model (with minimal consultant support) in managing new and follow-up cases of suspected keratoconus. AHPs, including optometrists and nurse practitioners, were able to make the same correct decision as the corneal consultants in 92% of the cases! Last week I had the privilege of being invited to present at the Australian and New Zealand College of Anaesthetists perioperative conference. In our session - Jacqueline Close presented data from the Australian & New Zealand Hip Fracture Registry that informs our (recently updated) Australian Commission on Safety and Quality in Health Care Hip Fracture Standard, and - I connected the dots to how the success of this #qualityimprovement initiative, might inform more broadly, ALL #injury care for older patients. It was a fascinating opportunity to reflect on an example of registry data, embedded and informing ongoing #improvement in patient focused clinical care pathways and components of clinical care. I reflected, that #hip #fracture is the ONLY #injury for which we have national clinical care standards, and jurisdictional and organisational variability in access, pathways and clinical care exist for most other injuries in Au and NZ. Not all #variation is bad, but I'm sure we can all agree, better visibility and standardisation (from point of injury) is required to: - reduce under-triage and optimise safe timely person centred care, whilst - minimising over-triage to ensure injury system capacity can meet demand. There is much work to do in injury management in Au and NZ - especially for older injured patients, with our most common mechanism of injury (including major trauma), a low force fall from standing height. The multidisciplinary "homes" for patient problems, are currently hard to see and the momentum to get there, difficult to build from within silo'd professions. #CIRSEtakeover with Dr. Joo-Young Chun Inpatient ward rounds are the easiest way to begin #ClinicalPractice in IR. Seeing patients on the ward is an important part of daily practice for all IRs and is critical to promoting IR as a clinical specialty. Speaking with anxious patients and concerned relatives is a skill that requires practice, and your communication skills will be tested. Clinical skills of taking a focused history and performing relevant physical examinations also require refinement that will improve with time. It is crucial that IRs are seen in hospital wards, intensive care units and emergency departments. We need to be by the patient’s bedside to understand their clinical status and have meaningful face-to-face discussions with clinicians. We need to be present for clinical decision-making - this is the only way we will be seen as clinicians.
