Well worth a read the standards mentioned and watch the video. Super to see real alternatives to hospital in action. These ideas are not new in the early 90s community rehab teams and intermediate care had similar positive impacts. They do need to be at scale however and require significant resource shift. It’s a pity not all areas in Wales are yet to grow the community services and degree of integration as illustrated here. There are particular problems also when patients are in acute English hospitals and need rehab or support to return to Wales. super post however very well done to all responsible. Finally I’d advise those commissioning such services not to assume that just collocating staff is the answer. To have high performing services requires leaders from all professions to truly embrace MDT clear roles common goals.
