Leading European Hospitals are implementing H2O outcome sets We are over the moon about a very important milestone for making patient-centric healthcare in Europe a reality. Members of the European University Hospital Alliance (EUHA) are rolling out H2O outcome sets to collect patient-reported outcomes in a standardized way. Together, we have defined standards for collecting patient-reported outcomes and testing them in the clinical day-to-day. What has been a vision for long, is now becoming reality as the project is entering the implementation phase.   Among the first to implement H2O outcome sets are the Charité - Universitätsmedizin Berlin and the MedUni Wien. Already two clinics within Charité – in the area of IBD and diabetes - are using the so-called H2O pragmatic sets, aiming at keeping the effort around the collection process itself as low as possible, for patients and clinicians. Also, the Medical University Vienna has started to use the H2O outcome sets in their diabetes clinic for collecting PROs. A major milestone not only for patients and clinicians, but also for researchers all over the world. Do you know what the PRO-HOME Project is? In this study, we tested the efficacy of a multicomponent intervention (cognitive, physical, educational, and relaxation activities) held inside a technologically monitored intrahospital facility in reducing the length of hospital stay and multidimensional frailty in hospitalized older people. Our patients reported a 2-day reduction in length of hospital stay and moved from the MPI-2 category of risk (moderate risk of frailty) to the MPI-1 class (low risk of frailty)! ViPHS (Video support in the PreHospital acute Stroke chain); Our first paper on this telemedicine solution to enable faster thrombectomy has now been published. ViPHS enables more equal care as even those who do not live near thrombectomy centers, typically University Hospitals, are now given a better opportunity for faster optimal care. This can be of vital importance for the individual patient, e.g. in terms of outcomes and lifelong functional disorders. Professor Lars Rosengren and I took the initiative together for ViPHS in 2015 and soon the core of people behind the published paper was established. The paper presents the work with prototype and full-scale care simulations incl. ambulance, consultation and realistic patient cases (actors). The following parties were involved in the work: Chalmers Univ. of Technology, Sahlgrenska University Hospital, Sahlgrenska Academy, PICTA, Prehospen at University of Borås, Sjukvårdens Larmcentral in Region Västra Götaland (prio and dispatch), Ambulance service at SU and Ambulance service in Södra Älvsborg Hospital. The ViPHS project is currently being run by the Region Västra Götaland as part of their "Digitalization" initiative. The Top-Ranked Heart Hospitals in the Great Lakes Region: Among the top 50 rated U.S. hospitals for adult heart care in the 23-24 U.S. News & World Report survey, 9 were based in the 5-state Great Lakes region. The region's TOP heart hospital, Cleveland Clinic, was also rated as the top adult heart hospital in the United States. Monitoring of quality of care is pivotal to identify gaps between guideline-recommended interventions and actual care in clinical practice. The SWEDEHEART registry has audited quality of secondary preventive care in Sweden since 2005. Most variables represent patient-level outcomes, showing considerable variation in target attainment between cardiac rehab (CR) centres, for which reasons remain unclear. To better understand these inter-centre differences we have now implemented new variables to audit centre-level structure and processes applied at CR centres around the country. Whether audit and feedback of this kind can aid centres in improving guideline adherence will also be evaluated as a part of the Perfect-CR Implementation study. Please join us in congratulating attending physician Dr. Ana Sofia Warner on her recent New Faculty Excellence Award from the Boston University Medical Group! In her nomination, her colleagues described her as, "a true bridge-builder and innovator". Dr. Warner created and launched BMC's Hospital Admission Reduction Program (HARP), which provides community-based transitional care to patients 65 and up who have been hospitalized at BMC. 💭 What does “excellence in care” mean to you? For me, it means trying to give the best care to the patients I see in clinic. It also means trying to improve care for patients I may never meet, through programs like HARP and partnerships with different groups and departments. 💭 How is BMC pushing the boundaries of care? I think there are a lot of folks at BMC who are very creative thinkers in terms of how we can engage with patients in our community. I see this every day with the amazing members of the HARP team, they go the extra mile for patients and that makes a big difference. 💭 What does rewriting healthcare mean to you? It means thinking outside the box of how we can engage patients to meet them where they are at. It means not being so tied to traditional ways of treating patients. That’s why our HARP program goes to people's houses. We realized we needed to be proactive after patients were discharged from the hospital, or they would end up coming back into the Emergency Department. 💭 How are you providing equitable and excellent clinical care? I speak Spanish, and I think for some patients it's a big relief to see a physician who speaks their own language. In my work at the Belkin Breast Center, there's a lot of nuance in some of the breast cancer counseling, so I feel like being bilingual is especially important. 💭 What’s special about working at BMC? I get to work with really smart, talented, and dedicated people every day. And we get to try to tackle some really hard problems in health care for patients who are often underserved.
