During #NHCW24, HFP is excited to highlight a recent pilot program from Spectrum Health Services, Inc. that focuses on the synergistic effect of integrated care and partnerships. Spectrum's collaboration with Get Fresh Daily (https://lnkd.in/eeeuebGD) on a “Health Produce Prescription Pilot Program” showed promising preliminary results. The program addressed both social influencers of health and health behaviors for a cohort of patients with two or more chronic conditions. During the 16-week program: - Participants received local/organically grown produce boxes weekly for free, along with recipes. - Participants met with members of their care team that included a medical provider, behavioral health consultant, and dietician at least three times. - Participants were encouraged to attend monthly cooking classes. At the end of the 16-week program, there were notable changes in participants’ health and health behaviors. Significant outcomes included improved diabetes and hypertension control and improved cholesterol for many participants! The program was primarily designed and coordinated by Hilary Reichlin, MSW, LCSW, the Director of Integrated Services at Spectrum, with Jiana Murdic, Founder of Get Fresh Daily, as its primary collaborator. I couldn't agree more with Dr. Burks in this article about nomenclature of "value-based care". Value is obscure and depending on who or what you are talking about can mean a lot of things to a lot of different people. I spend a lot of time explaining what this term means to folks who work in the space already so I am down for change. "Healthy-based care" is self-explanatory and people centered so I may try it our as I work with clinical teams. Let me know what you think, do you like the term value-based care, or should healthcare adopt the term health-based care. To improve health outcomes for seniors after hospital discharge, Rebecca Trotta and colleagues have been reimagining hospital-to-home care transitions with a model called SOAR (Supporting Older Adults at Risk). Drawing on the Discharge to Assess model, SOAR involves coordination among a core team specializing in geriatric care and an interprofessional team over three phases: Prepare, Transition, and Support. Pilot results, reported in the Journal for Healthcare Quality, demonstrate success! On average, SOAR increased the percentage of patients discharged before noon, from 6% to 76%, reduced the time to first home care visit from 49 hours to 6 hours, and cut down hospital length of stay by more than a day. It also led to to improvements with 30-day ED visits and readmissions and facilitated connection to the in-house pharmacy, which allows patients to obtain medications before discharge. Many thanks to all the participants for our 29th edition of #IPHS-online PH Roundtable and Educational forum. Key focus was on Patient Centered Care (PCC): Components, Principles, Practices and Current trends. Some key takeaways. 1. More than 8m people die every year from conditions that are treatable by the health system and 60% of these deaths are due to poor quality care. 2. High quality systems could prevent 2.5m deaths from Cardiovascular diseases, 1m newborn deaths, 900k deaths from TB and 50% of materbal deaths each year. 3. The Institute of Medicine defines PCC as Providing Care that is respectful of and responsive to the individual patient preferences,needs and values and ensuring that patient values guided all clinical decisions. 4. A report from Lancet Commission shows: 1in 3 people in LMIC cited negative experiences in health system in the areas of attention,respect communication and length of visit. Quality of care is worst for the most vulnerable, poor,less educated,adolescents and those with stigmatized conditions. As healthcare workers we must look out for and show extra care for the voiceless and helpless 5. #PickersEight Principles of #PatientCenteredCare: #Respect for patients preferences #Coordination and Integration of Care #Information and Education #Physical comfort #Emotional support. #Involvement of family members #Continuity and Transition #Access to care. 6. In my latest addition to the on-going debate about value assessment, I wrote a commentary with Richard Willke, Laura Pizzi, and Peter Neumann re-affirming the importance of measuring how much health a treatment, like a prescription drug, provides to patients. We discuss quality-adjusted life years (QALYs), the move to include alternative measures, and push back against the blunt arguments that QALYs discriminate against people with disabilities, like chronic conditions, and the elderly. Clarifying the terms and how and why to use value assessment and cost-effectiveness analysis continues to be important work towards ensuring patient access to affordable, effective medicines. https://lnkd.in/eDgqcrQW

Great job completing the NTM/MAC survey! 👏👏👏 I recently learned just how crucial these surveys are. The FDA requires patient input before researchers can move forward, ensuring "patients' experiences, perspectives, needs, and priorities are meaningfully incorporated into decisions about their health." This empowers patients! I'll share from experts as I learn more, but this is how we make a difference. Some of us were ineligible for the survey which can be frustrating. We want to contribute, and being told "no" feels like rejection. Often, researchers need very specific patient groups, so please don’t take it personally. Perhaps at some point, surveys will not only message us saying we’re ineligible, but might even explain why. This would certainly make us feel better and encourage us to participate in the future. The more we participate, the faster research advances. Imagine if research were constantly stalled due to lack of participation by patients - treatments for rare diseases could become too expensive to pursue. We must show that we are ready and willing to contribute and for many, surveys and interviews are comfortable ways to do so. While I love discussing exercise, airway clearance, and healthy eating, my focus will increasingly be on research and advocacy. Remember, progress requires participation.
