Fluid resuscitation is a crucial aspect of emergency and critical care. Since the advent of the concept of early goal-directed therapy, we have placed a huge emphasis on aggressive fluid resuscitation in patients with severe sepsis and septic shock. From EGDT to PROCESS/ARISE/PROMISE to Surviving Sepsis Guidelines, we have seen a shift in how fluid resuscitation is monitored, but the idea of aggressive fluid resuscitation is still the crux of our hemodynamic management of these patients. Yet, the FENICE study showed that in 46 countries, there is a “huge variability in the current practice regarding an FC [fluid challenge]…and may reflect the controversies in current guidelines.” (Cecconi 2015)

Wrapping this week once more with an interesting case worth Sharing...... A Challenging ECG Case: Lessons Learned and Shared I recently encountered a fascinating ECG case at the hospital that I'd like to share with my fellow healthcare professionals. A 65-year-old male patient presented with chest pain and shortness of breath. The initial ECG showed significant ST-segment elevation in leads II, III, and aVF, indicating an inferior wall myocardial infarction. Based on the results, we activated the cath lab and performed an emergency coronary angiogram, followed by primary percutaneous coronary intervention (PCI). However, the patient's condition evolved over time, revealing a complex anatomy with multiple vessel disease. This led us to adjust the treatment plan and perform additional stenting procedures. - The importance of serial ECGs in monitoring the progression of myocardial infarction and detecting potential complications. - Considering alternative diagnoses, such as aortic dissection or pulmonary embolism, in patients presenting with chest pain and ECG abnormalities. _Conclusion:_ This case highlights the importance of vigilant monitoring and interdisciplinary collaboration in managing complex cardiovascular cases. The Texas Heart Institute, #BiVACOR®, CHI St. Luke's Health - Baylor St. Luke's Medical Center, and Baylor College of Medicine completed the first successful human #implantation of the BiVACOR Total Artificial Heart (TAH) on July 9, 2024. The procedure, performed at Baylor St. Luke’s Medical Center, showed no complications and excellent performance, and four more patients will be enrolled in this human clinical study. Heart failure affects over 26 million people globally, and those experiencing #heartfailure have severely limited #transplant options.
