The latest Patient Safety Culture Hospital Survey from the Agency for Healthcare Research and Quality provides new insight into healthcare providers’ assessment of patient safety during handoffs and information exchange. During shift changes, 71% of providers and staff respondents agreed there’s adequate time to exchange all key patient care information. Yet 63% of staff agreed important patient care information is often omitted during shift changes and 56% of staff agreed information is left out when transferring patients from one unit to another. The results show that most providers and staff are finding opportunities to exchange information, but there seems to be an issue with the communication of information itself. Standardizing clinician communication and establishing a structured handoff framework prevents communication failures that lead to adverse events and patient harm.
