In March 2020, Calvin Meaney, PharmD, interim division head, Division of Outcomes and Practice Advancement and clinical associate professor, #UBuffalo Department of Pharmacy Practice, met with a group of nephrology pharmacists to discuss ways to help improve patient lives and keep the nephrologist at the center of the patient care team. From those discussions, the Advancing Kidney Health through Optimal Medication Management (AKHOMM) initiative was formed and Meaney now serves as an assistant director. The vision of AKHOMM is every person with kidney disease receives optimal medication management through team-based care, including a pharmacist, to ensure their medications are safe, effective and convenient. Recently, Meaney and the AKHOMM team concluded that pharmacist-led optimal medication management is achievable within a value-based care model. This approach involves pharmacists taking a central role in managing patient medications, which improves medication adherence and patient health outcomes while also reducing overall healthcare costs. They found integrating pharmacists into nephrology and primary care practices can help empower patients to be partners in their medication management which can help decrease nephrologist workloads. As some of you know, I have been editing the Peripheral Brain for Pharmacists for many years. Each year, we work hard to update the book, guidelines, and current practice-related resources and add new content. Making quick reference sheets and consolidating information started when I was a rotation student as a memory aid—flash forward, and I get to share that. Each year, I'm impressed with the different materials that are put together. But this year, wow! I love this video below and it's cool to aid digital elements to the mix. Yale Collaboration for Regulatory Rigor, Integrity, and Transparency (CRRIT): a Research & Policy Collaborative between the Yale School of Medicine, Yale Law School, and Yale School of Public Health.
Yale CRRIT in Health Affairs Forefront offers the FDA suggestions as part of the #AdComm reform process for the three areas identified in their recent listening session. The first area identified focused on advisory committee composition, the second was advisory committee service as special government employees, and the third was public perception and understanding of advisory committees. In the piece, the CRRIT team outlines several recommendations for reforming advisory committees to achieve the agency's goals of "protecting & promoting public health and preserving public trust," including: - Greater involvement from patients and caregivers, as well as generalist clinicians - Enhanced transparency and management of financial COI to ensure truly independent advisory committees - Mandating transparency of financial COI of open public hearing participants -instead of relying on self-disclosure and restricting future participation if they fail to do so - Standardize ethics rules for advisory committee members who serve as special government employees across the FDA Divisions or Centers, including barring those over the past three years with direct financial COI with product sponsor to not serve on advisory committees or those with non-research ties to competitors - Preserving the advisory committee vote as a critical signal to the FDA and the public on the strength of the recommendation informing a regulatory decision - Using public communications to clarify how the advisory committees vote informed the FDA's final decision (and that, yes, advisory committees are advisory). In their new Forefront article, Reshma Ramachandran, Joseph S. Ross, Julia S. Etkin, and Jason L. Schwartz from Yale School of Medicine, Yale New Haven Hospital, and Mayo Clinic discuss how there are a number of key opportunities for the FDA to maintain and enhance engagement with independent expert advisory committees to achieve the agency’s goals of protecting and promoting public health and preserving public trust. "The FDA currently has 50 technical and scientific advisory committees spanning disease areas and medical product types as well as cross-cutting domains such as pediatrics, digital health, and risk safety communications. Each committee primarily comprises scientific experts including basic science and translational researchers, clinicians, engineers, and other technical specialists. Other members include patient, consumer, and industry representatives, the latter of which are non-voting members. The FDA convenes these members to deliberate specific questions posed by the agency. For the recent listening session, the FDA asked which views, voices, and perspectives would or would not be important to be represented on the advisory committee."
