🎉 Wrapping up November! 🎊 Hey LinkedIn folks, As we say goodbye to November today, it's a moment to look back on 2023's journey. Tomorrow marks December, the final stretch of the year. Taking a second to highlight the impact of integrated care systems. This year showed us how these systems, bringing together different healthcare elements, can make a real difference. They break down old walls, making sure patient care is smooth and centered on their needs. Entering December, let's keep pushing for integrated care. It's more than a trend; it's a change that can shake up healthcare and make life better for many. Cheers to the last month of 2023, time for thinking, being thankful, and keeping the healthcare evolution going! #December1st #IntegratedCare #HealthcareJourney #YearEndVibes App for cancer patients launches in Tameside and Glossop Cancer patients in Tameside and Glossop now have a convenient way to receive personalized advice and support, thanks to a recently launched app. Introduced in September 2023, the Tameside and Glossop NHS ICFT’s Patient Portal serves as a comprehensive hub for cancer-related health and wellbeing information. Alliance Healthcare's game-changing move! 🩺💡 Are you familiar with the alarming reality that cancer touched the lives of approximately 3 million people in the UK last year? This reality prompted Alliance Healthcare to launch a proactive step in cancer care - the 'Not Normal for You?' referral scheme. Now accessible in UK pharmacies, this initiative empowers pharmacists to be the first line of defense, identifying potential cancer symptoms and facilitating swift referrals to GPs. 🚨 Imagine walking into your local pharmacy with a niggling health concern - that's where the expertise of these trained professionals comes into play. With their keen eye for early cancer signs and 'red flag' symptoms, pharmacists are now equipped with educational materials and those vital orange NNFY referral cards that could guide your GP in diagnosing cancer early. 📝 Why is this significant? Early detection often means more treatment options and better outcomes. Innovations like these are transforming community pharmacies into pivotal healthcare hubs, reinforcing the critical role pharmacists play in our healthcare ecosystem. Let's applaud 👏 Alliance Healthcare for harnessing the power of their network and for prioritising our health. Remember, your local pharmacist might just be your unsung hero in the fight against cancer. #HealthcareInnovation #CommunityPharmacy #CancerAwareness

#cancerjourney - I know many of you follow me because of my blog the #theferrymans column. Unable to speak because of cancer treatment the best way to communicate with family and friends in the nondigital age was to write. Last night I penned a new entry. I share that entry with you of the latest episode of my never-ending journey post-cancer treatment. I share this as a support for others who have, are, and will be thinking the same things as I do. Data is my life. My treatment regime is being undertaken at the Royal Devon University Healthcare NHS Foundation Trust. There was a surreal experience walking down corridors full of patients that I had pushed patients down at 4 am as an HCA in the Emergency Department. Whilst working at the Hospital the Trust introduced a software patient management system called #Mycare powered by Epic. It gave the patient journey a single view. If I had to admit a patient a paper trail would start and end some 30 mins later. My Care revolutionised data management and patient care allowing us to spend more time with the patients. Yesterday I saw the flip side. Patients can now log in to the My Care App and see their records and notes, medicine, and appointments. The system provided me with my patient journey from the start to the next stage providing reassurance that I am in the system and full transparency exists. It is simply brilliant for someone who has seen both sides of the proverbial coin I now buy the power of health data and the need for the right safeguards which will revolutionise patient care. Well done Royal Devon University Healthcare NHS Foundation Trust for a brilliant cancer pathway and also improving my care from 18 years ago through great communications and of course brilliant clinicians. If anyone needs to talk I am here. #keepattacking https://lnkd.in/dEM7VwrH

https://lnkd.in/evCca6zg Long time for me on working with HEDIS Measures but I loved seeing this 2025 news for women's health for 40-74 (breast cancer) and reporting requirements of the Breast Imaging Reporting and Data System (BI-RADS): Documented Assessment After Mammogram*. The percentage of episodes for members 40–74 years of age with mammograms documented in the form of a BI-RADS assessment within 14 days of the mammogram. The percentage of episodes for members 40–74 years of age with inconclusive or high-risk BI-RADS assessments that received appropriate follow-up within 90 days of the assessment.

Cancer Care Reviews increased by 60% last year in the Upper Calder Valley Primary Care Network (PCN) thanks to upskilling a Social Prescribing Link Worker in the knowledge of cancer, an understanding of the cancer care pathway and how to support patients with a cancer diagnosis. Cancer Care Reviews are a requirement of the Quality Outcomes Framework that all GP Practices must adhere to, and the pilot trialled in the Upper Calder Valley aimed to improve the uptake and patient experience of reviews by identifying alternative members of the healthcare team who could offer support to patients in a more holistic way. Training included the development of an education plan, competency framework and shadowing clinical teams within secondary care to ensure Care Coordinators and Social Prescribing Link Workers were equipped with what they needed to appropriately support patients. Appointment slots with Social Prescribing Link Workers and Care Coordinators have the potential to be longer than with a GP, benefiting from an established vast knowledge of community services and supportive networks. The pilot aimed to utilise this knowledge to ensure individuals with a cancer diagnosis also have their non-clinical concerns addressed, thereby ensuring they can access the invaluable holistic support available from their local communities. ➡ Read all about it including feedback from patients here: https://lnkd.in/gmb_aV78 👏 Thank you to Dinah Coggon for your support with this pilot and the rollout across all #Calderdale PCNs #CancerCareReviews #CancerCare #PrimaryCare #PrimaryCareNetwork #Calderdale #WestYorkshire West Yorkshire and Harrogate Cancer Alliance, West Yorkshire Health and Care Partnership, Calderdale and Huddersfield NHS Foundation Trust

LinkedIn Top Voice | CEO, Cosmo Pharmaceuticals | Harnessing A.I. to Improve Patient Outcomes | Health Equity Advocate | Empowering Teams
Our healthcare system in the United States is broken in a lot of ways. Getting a cancer diagnosis shouldn't mean that you go into debt for the rest of your life. When you're trying to fight cancer, the last thing you should be worried about is whether or not you can afford it. This is why health equity is so important to me. When we tackle inequities, we see better patient outcomes. “Cancer survival rates over recent years have shown improvement across races and ethnicities, so by acknowledging and addressing barriers to care, further improvements toward equitable access, treatment, and outcomes can be realized for all,” says Lori Pierce. If we want to see even more positive change when it comes to cancer care, we have to see an embrace of health equity throughout all medical care. It's also important to note that we can't only focus on increasing accessibility to care... we have to also make sure that care is unbiased and that healthcare providers are aware of inequities that affect medical care overall. #CancerCare #HealthEquity #Healthcare #CancerPreventionMonth

Fridays are more than just the end of the workweek – they're your chance to reflect on your victories, set fresh goals, and gear up for a weekend recharge. 🚀 Here's to an awesome Friday full of positivity, productivity, and the mojo to finish the week on a high note! 💼✨ #FridayFeeling #WeekendVibes #CelebrateSuccess ✨Transforming Cancer Care: A Vision for Early Detection and Improved Outcomes✨ Cancer survival rates are on the rise, with an 11% increase in one-year survival for patients diagnosed in 2015 compared to 2000. The NHS aims to further enhance these outcomes by diagnosing the majority of cancers at stages 1 and 2 by 2028. The Long Term Plan outlines a comprehensive strategy, emphasizing early diagnosis through awareness campaigns, lowering referral thresholds, and accelerating access to treatment. Key initiatives include modernizing the Bowel Cancer Screening Programme, introducing HPV primary screening for cervical cancer by 2020, and extending lung health checks nationwide. A faster diagnosis standard from 2020, along with the introduction of Rapid Diagnostic Centres, promises quicker access to accurate diagnoses. In summary, the NHS is dedicated to transforming cancer care, with the goal of diagnosing 75% of cancers at stages 1 or 2 by 2028. This ambitious vision reflects a collective commitment to early detection and improved outcomes for cancer patients. #CancerCareRevolution #EarlyDiagnosisMatters #TransformingCancerOutcomes #NHSProgress #HealthcareInnovation #CancerSurvivalGoals #MedicalAdvancements #PatientCentricCare #CancerAwareness #FutureOfHealthcare #NHSVision2028 #InnovateForCancer #BetterTogetherAgainstCancer #HopeForSurvivors #HealthEquity #EarlyDetectionSavesLives #PositiveImpactHealthcare #CancerPrevention #PatientEmpowerment #HealthcareTransformation For more information: https://lnkd.in/euW3uiBU

For employers and labor unions, who provide healthcare coverage for nearly half of all Americans, supporting the entire cancer care journey is no longer just nice to have—it’s a necessity. That’s why we’re expanding to an end-to-end comprehensive cancer solution, in partnership with the American Cancer Society. To impact outcomes and cost, employers should look to take a two-pronged approach to cancer management: 1⃣ Help employees catch cancer earlier through evidence-based and covered interventions. 2⃣ Navigate employees through treatment with comprehensive care from diagnosis to remission. Read more in our recent blog ➡️ https://lnkd.in/gyAKheUm

It’s National Women’s Health Awareness Week! This week, OVAC spotlights the importance of prioritizing women’s health, specifically in the fight against cancer. Cancer affects women in unique ways, with certain types of cancer posing higher risks. From breast and ovarian cancer to cervical and uterine cancer, the impact on women’s lives cannot be understated. That’s why OVAC is committed to advocating for greater federal investments in #CancerResearch and #CancerPrevention programs. Congress plays a critical role in shaping policies and appropriations that directly impact women’s health outcomes. By increasing funding for the The National Institutes of Health, National Cancer Institute (NCI), and Centers for Disease Control and Prevention, we can enhance early detection methods, develop more effective treatments, and ultimately save more lives. This National Women’s Health Awareness Week, let's empower the women in our lives to take charge of their health, especially regarding cancer prevention & treatment, by practicing healthy habits, getting routine screenings & vaccinations while OVAC will continue to urge Congress to allocate more funding in #FY25 for cancer research and prevention programs. Together, we can make strides towards a future where more women affected by cancer will have the resources and support they need.💗 #NationalWomensHealthAwarenessWeek https://lnkd.in/e2nY9ZPp
