Our care coordinators do an amazing job of ensuring that healthcare providers are up to date on their patient's well-being. Here's one story from Cynthia, a HealthXL care coordinator: Cynthia had a patient whose readings were very sporadic and unusual for them. She reached out to the patient to check on them and found that they were in the hospital for GI bleeding. She was able to alert their provider's office as they had recently prescribed a medication that could have contributed. Cynthia took the necessary steps to ensure the provider was informed on the patient's well-being and provided them with the vitals log containing the patient's readings.
