We’ve been providing cost consultancy services on this exciting scheme, which will create a new suite of operating theatres at the hospital. Did you know that operating theatres are 3 to 6 times more energy intensive that the rest of a hospital? Wow! The NHS in Scotland is working hard to make theatres greener with its National Green Theatres Programme. I was glad to help in a small way by reviewing this report and its recommendations moving forward. Key messages and my interpretations in brackets (which apply to much more than just theatres): 1.There is limited published evidence (so we have to work to get it by doing things differently and sharing our learning) 2. A reduction in resource use (stuff we use) can mean higher up front costs in the short term (but cost saving in future so let's plan longer term and invest in sustainable care by doing things differently with fewer single use products) 3. Primary prevention of surgical conditions can greatly reduce the need for surgery (prevention is better than cure and cutting into people, teach people how to live their lives in the best of health with a National Healthy Service rather than a National Illness Service) 4. Small changes can lead to environmental sustainability improvements (every little counts, switch off when not in use, don't use or unpack what you don't need) 5. Barriers to improvements in practice include lack of leadership, workload pressures and limited knowledge (step up, share what works, listen to staff who know the system, think "what if we could" and do it!) NHS could generate between £2.18 billion and £6.55 billion annually Current Operating Hours: NHS theatres typically run 10 hours per day, from 8:00 AM to 6:00 PM, Monday to Friday. Extended Hours Proposal: Extending to 14 hours per day (8:00 AM to 10:00 PM), adds 4 extra hours per weekday. Additionally, opening theatres for 8 hours on Saturdays (since they are usually closed for elective surgeries). Extra Hours Calculation: Total extra hours per week: 20 hours (weekdays) + 8 hours (Saturday) = 28 extra hours per week. Total extra hours per year: 28 extra hours per week x 52 weeks = 1,456 extra hours per year per theatre. Nationwide Impact: With 3,000 operating theatres in the UK, this adds: 1,456 hours per theatre x 3,000 theatres = 4,368,000 extra hours per year nationwide. Revenue Potential: If these 4,368,000 extra hours were rented to the private sector at £500 to £1,500 per hour, the NHS could generate between £2.18 billion and £6.55 billion annually. Add to it that all the capital expenditure that is currently done by the private health system, building hospitals etc would be reduced, plus reduced rick for private health to have to be cash rich to create assest, so many more groups could appear to create private health groups, thus reducing the cost of private operations. Thus meaning more people would do elective private surgery thus removing burden from the NHS. Add to this these private health operations would be outside of core working hours and thus reduce the "time off work" for minor operations.

Are we letting hearts wait too long? Exploring NSW's delay in adopting life-saving TAVI technology. Edwards Lifesciences is addressing aortic valve stenosis, currently Australia's most prevalent heart valve issue, through a campaign promoting minimally invasive TAVI in New South Wales. TAVI offers a quicker recovery and lower costs compared to traditional surgery. Despite TAVI's inclusion in the Medical Benefits Scheme since July 2022, NSW lags in adoption, leading to ethical concerns and unequal access. Waiting times for TAVI in NSW vary from 3-4 weeks in the private sector to 6-12 months in the public sector. According to the National Library of Medicine in Australia, "If left untreated, symptomatic severe aortic stenosis has an extremely poor prognosis, with a 30–50% mortality at 12 months." Alarmingly, NSW public hospitals treat less than 1,000 cases per year. At the same time, approximately 32,000 residents live with severe aortic stenosis in the state, with over half projected to die within five years without intervention. I know this all may look bleak but having such an honest appraisal of the NHS is exactly what we need. Let’s use this report as a positive and create solutions to future proof a healthcare system that is the envy of many. We look forward to Keir Starmer and Rt. Hon. Wes Streeting MP responses and their 10 year plan to adopt innovation such as ours to drive increased productivity. Infix Support are delivering tools to the NHS workforce to ease the pressures they are under. As Professor the Lord Darzi of Denham OM KBE PC FRS rightly points out we are working harder than ever but the productivity has fallen. However, our workforce have unnecessary burdens of excessive administration, manual processes and outdated equipment that don’t allow clinicians to deliver the care they want to patients. Time to bring our treasured NHS into the modern digital age.
Following the publication of the Professor the Lord Darzi of Denham OM KBE PC FRS report, our main takeaways at Infix Support are that we cannot afford to lose the NHS but immediate action is needed. Central to the report is the need to increase productivity and move from analogue to more digitally-enabled NHS. The rollout of infix's digital scheduling software across NHS Scotland health boards has vastly improved the administration process through automation and intelligent theatre scheduling processes, with increases in operating theatre productivity up to 20%.
