Medicaid, the largest source of HIV care funding in the US, must take crucial steps to harness the potential of breakthrough, longer-acting products for HIV prevention and treatment.
In their Big Ideas Brief, experts from the O'Neill Institute for National and Global Health Law, Amida Care, and CAI (Cicatelli Associates Inc) TAP-in project give concrete strategies and recommendations for state and federal officials to ensure access to these game-changing medications, with a focus those populations most deeply impacted by HIV.

Welcome to the lecture on the path towards an HIV cure by Steven G. Deeks, MD. Dr. Deeks is a Professor of Medicine in Residence at the University of California, San Francisco (UCSF) and a faculty member in the Division of HIV, Infectious Diseases, and Global Medicine at Zuckerberg San Francisco General Hospital. An internationally recognized expert on HIV pathogenesis and treatment.
Dr. Deeks is a member of the American Society for Clinical Investigation (ASCI) and the Association of American Physicians (AAP).He is listed annually as one of the world’s most cited scientists. In 2022.
Content of Lecture
1. Long-term health in the absence of therapy ("functional cure")
Cancer model (remission)
Occurs in 1% of natural infections and may be occurring in recently identified "post-treatment controllers" (e.g., Visconti Cohort).
Will there be residual disease?
Approach: Enhance HIV-specific immunity.
2. Complete eradication of all replication-competent HIV
Is this remotely possible?
Is this necessary?
How can this be proven?
Approach: Induce transcription of latent HIV genomes in resting CD4+ T cells during completely effective antiretroviral therapy.
3. Conclusions
A safe, scalable cure may prove impossible and will take years to decades to develop even if possible, but there are reasons to be optimistic.
Barriers to advancing the cure agenda include:
Current ART is not fully suppressive in many, perhaps most, people.
No sensitive, high-throughput assay of relevant reservoirs exists.
