Public policy advocacy is a critical backbone of quality clinical care. Organized advocacy such as Neurology on the Hill is one small way that physicians target improved clinical care for patients in healthcare. This article provides pertinent updates on Neurology on the Hill. Reports indicate a focus on the Safe Step Act, Medicare cuts, and research funding for neurology in 2024, however a range of issues are addressed during legislative discussions. Understanding the Congenital Gerbode Defect: A Comprehensive Review In the intricate field of congenital heart defects, few conditions pose as significant a diagnostic challenge as the congenital Gerbode defect. This rare anomaly, characterized by an abnormal communication between the left ventricle and the right atrium, demands a nuanced understanding and a meticulous approach to diagnosis and treatment. Our recent review, "Congenital Gerbode Defect: A Left Ventricular to Right Atrial Shunt—State-of-the-Art Review of Its General Data, Diagnostic Modalities, and Treatment Strategies," provides an in-depth analysis of this condition, shedding light on its clinical implications, diagnostic hurdles, and therapeutic strategies. Unveiling the Gerbode Defect Despite its rarity, the congenital Gerbode defect presents a significant clinical challenge due to its diverse range of symptoms. From dyspnea and palpitations to growth retardation and asymptomatic cases, the clinical presentations are varied, complicating the diagnostic process. Our review consolidates findings from 78 reports, offering a comprehensive understanding of the defect. Key Diagnostic Approaches A robust diagnostic approach is crucial, particularly in newborns. The following modalities are recommended: Auscultation: This initial non-invasive technique remains fundamental. Transthoracic Echocardiography (TTE): Essential for visualizing the defect. Transesophageal Echocardiography (TEE) and Magnetic Resonance Imaging (MRI): Optional but beneficial for detailed assessment. The diagnostic process is complicated by the Gerbode defect’s similarity to more common cardiac conditions. Therefore, pediatricians and cardiologists must maintain a high index of suspicion to ensure early and accurate diagnosis. Treatment Strategies: Surgical vs. Device Closure Once diagnosed, early and individualized treatment is imperative to prevent long-term complications. Our review suggests a preference for surgical defect closure over device closure based on current evidence, though significant advancements are being made in device closure techniques. Early intervention can significantly enhance the patient’s quality of life and mitigate potential adverse outcomes. For healthcare professionals, staying informed about these developments is crucial.
